INDICATION

Danzol 100mg Cap is a synthetic androgen and anterior pituitary suppressant indicated for the treatment of endometriosis, including management of pain and reduction of endometriotic lesions in women with mild to moderate disease. It is also indicated for fibrocystic breast disease and hereditary angioedema (both the prevention of attacks in males and females). At lower doses, it may be used for premenstrual symptoms.

MECHANISM OF ACTION

Danazol is a synthetic attenuated androgen derived from 17-alpha-ethinyl testosterone. It suppresses the hypothalamic-pituitary-ovarian axis by inhibiting pituitary gonadotrophin secretion (LH and FSH), reducing the midcycle LH and FSH surge and creating a chronic anovulatory state. This pseudomenopause creates a hostile hormonal environment for endometriotic tissue, which regresses in the absence of cyclical estrogen stimulation. Danazol also directly inhibits ovarian steroidogenesis by blocking multiple enzymes in the steroid biosynthesis pathway, reducing estrogen production. In hereditary angioedema, it stimulates the hepatic synthesis of C1-esterase inhibitor.

DOSAGE AND ADMINISTRATION

•      Endometriosis: 200 to 800mg per day in divided doses for 3 to 6 months; begin during menstruation

•      Fibrocystic breast disease: 100 to 400mg per day in divided doses for up to 6 months

•      Hereditary angioedema: 200 to 600mg per day; reduce to the lowest effective maintenance dose

•      Always follow the dosage and duration prescribed by your gynecologist or specialist

SIDE EFFECTS

Common:

•      Weight gain and fluid retention

•      Amenorrhea and menstrual irregularity (expected therapeutic effect for endometriosis)

•      Acne and oily skin (androgenic side effects)

•      Hot flushes (hypoestrogenic effect)

Less common:

•      Virilization: voice deepening, hirsutism, clitoral enlargement (dose-dependent androgenic effects; more common at higher doses)

•      Elevated liver enzymes and cholestatic jaundice

•      Adverse lipid profile changes (reduced HDL, increased LDL)

•      Benign intracranial hypertension (pseudotumor cerebri; rare)

DRUG INTERACTIONS

Danazol inhibits CYP3A4 and increases plasma levels of ciclosporin, tacrolimus, and warfarin; monitor levels and coagulation. Carbamazepine levels may be increased. Statins have increased myopathy risk when combined with danazol. Oral hypoglycemics and insulin may require dose adjustment as danazol can increase blood glucose. Antihypertensives may be affected by fluid retention.
