INDICATION

Dan-D 1 Lac IU Cap is a high-dose vitamin D3 supplement used for the treatment of severe vitamin D deficiency, hypocalcemia, hypoparathyroidism, and metabolic bone diseases including osteomalacia, rickets, and osteoporosis. It may be prescribed as a loading or induction dose under medical supervision, particularly in patients with very low baseline vitamin D levels detected on blood tests.

MECHANISM OF ACTION

Cholecalciferol (vitamin D3) is the natural form of vitamin D. After absorption, it is hydroxylated in the liver to 25-hydroxyvitamin D and then in the kidneys to the active form 1,25-dihydroxyvitamin D (calcitriol). Calcitriol binds to nuclear vitamin D receptors in intestinal cells, bone cells, and kidneys, promoting calcium and phosphate absorption from the gut, regulating bone mineralization, and maintaining serum calcium homeostasis.

DOSAGE AND ADMINISTRATION

•      As directed by a physician based on serum 25-OH vitamin D levels

•      Loading dose for severe deficiency: 1 capsule (100,000 IU) once every 2 to 4 weeks for 2 to 3 months

•      Maintenance dose is typically switched to a lower-dose formulation after correction

•      Take with a meal containing fat to maximize absorption of this fat-soluble vitamin

•      Regular blood tests are required to monitor vitamin D and calcium levels

SIDE EFFECTS

Common:

•      Nausea, constipation (with excessive doses)

•      Headache

•      Elevated serum calcium (hypercalcemia) with overdosing

•      Increased thirst and urination

Less common:

•      Hypercalciuria (high urinary calcium) leading to kidney stones

•      Anorexia and weight loss from hypervitaminosis D

DRUG INTERACTIONS

Thiazide diuretics reduce urinary calcium excretion and may increase the risk of hypercalcemia when combined with high-dose vitamin D. Corticosteroids impair vitamin D's calcium-absorptive action. Cholestyramine and colestipol reduce intestinal absorption of fat-soluble vitamins including vitamin D.
