INDICATION
Cubriva 100mg Tab is a third-generation anticonvulsant indicated as adjunctive therapy for partial-onset (focal) seizures with or without secondary generalization in adults and adolescents aged 16 years and above with epilepsy. It is used when existing antiepileptic medications have not achieved adequate seizure control. Brivaracetam has a higher affinity for the synaptic vesicle protein SV2A than levetiracetam, providing greater anticonvulsant potency at lower doses.
MECHANISM OF ACTION
Brivaracetam is a selective and high-affinity ligand for synaptic vesicle protein 2A (SV2A), which plays a key role in synaptic vesicle exocytosis and neurotransmitter release at excitatory synapses. By binding to SV2A with high affinity, brivaracetam modulates the release of excitatory neurotransmitters, particularly glutamate, at synapses, reducing hyperexcitability and abnormal electrical discharges that underlie partial seizures. Unlike levetiracetam, brivaracetam also inhibits sodium channels and does not share levetiracetam's mechanism of blocking tetanus-toxin-induced epileptiform activity; these additional mechanisms may contribute to its broader anticonvulsant profile.
DOSAGE AND ADMINISTRATION
• Initial dose: 50mg twice daily; may increase to 100mg twice daily (200mg/day) based on response
• Maintenance dose: 25 to 100mg twice daily individualized based on seizure control and tolerability
• Take with or without food at the same time each day
• Never stop abruptly; taper gradually under neurologist supervision; dose reduction required in hepatic impairment
SIDE EFFECTS
Common:
• Somnolence and dizziness (most common)
• Fatigue
• Nausea and vomiting
• Psychiatric symptoms: irritability, anxiety, depression, and mood changes (less common than with levetiracetam)
Less common:
• Euphoric mood and feeling drunk (CNS effect)
• Decreased appetite
• Diplopia and blurred vision
• Psychiatric disorders including psychosis and suicidal ideation (monitor mental health; class effect of antiepileptics)
NOT RECOMMENDED IN
• Known hypersensitivity to brivaracetam or any pyrrolidone derivative
• Severe hepatic impairment (Child-Pugh C; dose must be reduced in moderate hepatic impairment)
• Patients with a history of psychiatric disorders require careful monitoring
• Pregnancy: insufficient data; discuss risk-benefit with neurologist and use effective contraception
DRUG INTERACTIONS
Rifampicin and other potent CYP inducers significantly reduce brivaracetam plasma levels by approximately 50%; dose increase may be needed. Carbamazepine combined with brivaracetam may increase carbamazepine-epoxide metabolite levels; monitor for carbamazepine toxicity. Alcohol and CNS depressants have additive sedative effects. Brivaracetam is a weak CYP2C19 inhibitor; may slightly increase phenytoin levels; monitor phenytoin if combined.
