INDICATION

Cova 80mg Tab is an angiotensin II receptor blocker (ARB) indicated for the treatment of essential hypertension in adults and children aged 6 years and above. It is also indicated for the treatment of symptomatic heart failure in adults when ACE inhibitors are not tolerated, and for the reduction of cardiovascular mortality in clinically stable patients with left ventricular failure or dysfunction following acute myocardial infarction.

MECHANISM OF ACTION

Valsartan selectively and competitively blocks the angiotensin II type 1 (AT1) receptor, preventing angiotensin II-mediated vasoconstriction, aaldosterone secretion, and renal sodium retention, thereby lowering blood pressure and reducing cardiac and renal afterload. Unlike ACE inhibitors, valsartan does not inhibit the breakdown of bradykinin, explaining the negligible incidence of cough with ARB therapy. The 80mg dose is the standard starting dose for most hypertensive patients and provides effective 24-hour blood pressure control with once-daily administration.

DOSAGE AND ADMINISTRATION

•      Hypertension: 80mg once daily; may increase to 160mg or 320mg if needed after 4 weeks

•      Heart failure: start at 40mg twice daily and titrate upward as tolerated

•      Take at the same time each day, with or without food

•      Always follow the dosage and blood pressure monitoring schedule prescribed by your cardiologist

SIDE EFFECTS

Common:

•      Dizziness and headache

•      Fatigue and back pain

•      Hypotension, particularly on first dose or in volume-depleted patients

•      Nasopharyngitis

Less common:

•      Hyperkalaemia, particularly in renal impairment, diabetes, or with potassium-sparing diuretics

•      Elevated creatinine and renal function worsening

•      Angioedema (rare; less common than with ACE inhibitors but not absent)

•      Hypotension in heart failure patients

NOT RECOMMENDED IN

•      Pregnancy (all ARBs are contraindicated in pregnancy due to fetal harm)

•      Concurrent use with Aliskiren in patients with diabetes or renal impairment

•      Severe hepatic impairment or biliary obstructive disorders

•      Known hypersensitivity to valsartan or any ARB

DRUG INTERACTIONS

Potassium-sparing diuretics, potassium supplements, and other RAAS agents increase Hyperkalaemia risk. NSAIDs reduce antihypertensive efficacy and may worsen renal function. Lithium clearance may be reduced; monitor lithium levels. Aliskiren combination is contraindicated in diabetics and renal impairment. Rifampicin and cyclosporine can affect valsartan levels through transporter inhibition.
