INDICATION

Corex-D Syrup 60ml is a multi-symptom cough and cold preparation indicated for the symptomatic relief of non-productive cough, nasal congestion, and associated cold symptoms. Dextromethorphan suppresses the cough reflex, chlorpheniramine relieves sneezing and rhinorrhea, ephedrine reduces nasal congestion, and sodium citrate acts as an expectorant to loosen respiratory secretions.

MECHANISM OF ACTION

Dextromethorphan is a non-opioid antitussive that acts on sigma receptors and NMDA receptors in the brainstem cough centre, elevating the threshold for cough reflex activation without causing respiratory depression or dependence at therapeutic doses. Chlorpheniramine is a first-generation H1 antihistamine that blocks histamine-mediated sneezing, rhinorrhea, and nasal itching while also providing mild antitussive effects through central mechanisms. Ephedrine is a sympathomimetic amine that acts on alpha-1 adrenergic receptors in nasal mucosal blood vessels, causing vasoconstriction and reducing mucosal oedema and nasal congestion. Sodium citrate promotes respiratory secretion thinning through mucolytic expectorant activity.

DOSAGE AND ADMINISTRATION

      Adults: 10ml three to four times daily; maximum 40ml per day

      Children 6 to 12 years: 5ml three to four times daily

      Do not exceed the recommended daily dose of dextromethorphan (120mg/day maximum)

      Do not take with or within 14 days of MAO inhibitor therapy; do not combine with other cough and cold preparations

SIDE EFFECTS

Common:

      Drowsiness and sedation (chlorpheniramine; significant in adults)

      Dry mouth, constipation (anticholinergic)

      Insomnia and palpitations (ephedrine)

      Nausea

Less common:

      Elevated blood pressure (ephedrine)

      Urinary retention (anticholinergic + sympathomimetic)

      Tachycardia and palpitations

      Dextromethorphan dissociative effects at very high doses (recreational abuse potential)

NOT RECOMMENDED IN

      Concurrent or recent (within 14 days) MAO inhibitor use (life-threatening serotonin syndrome and hypertensive crisis risk with dextromethorphan and ephedrine)

      Hypertension, cardiovascular disease, hyperthyroidism, or diabetes

      Children under 6 years

      Known hypersensitivity to any component

DRUG INTERACTIONS

MAO inhibitors are absolutely contraindicated with dextromethorphan (serotonin syndrome) and ephedrine (hypertensive crisis). Fluoxetine and paroxetine (CYP2D6 inhibitors) significantly increase dextromethorphan levels and serotonin syndrome risk. Beta-blockers interact with ephedrine; avoid combination. CNS depressants and alcohol potentiate chlorpheniramine sedation.

STORAGE INSTRUCTIONS

Store below 25°C in a cool, dry place away from direct sunlight. Keep tightly closed. Keep out of reach of children.
