INDICATION

Combivair Forte Cap is an inhaled combination product containing budesonide 200mcg and formoterol 6mcg, used in the maintenance treatment of asthma in adults and adolescents aged 12 years and above who are not adequately controlled on inhaled corticosteroids alone. It may also be used in COPD as a maintenance bronchodilator and anti-inflammatory treatment.

MECHANISM OF ACTION

Budesonide suppresses airway inflammation by inhibiting the activation of inflammatory cells and the release of pro-inflammatory mediators, reducing airway oedema, mucus production, and bronchial hyperresponsiveness. Formoterol stimulates beta-2 adrenoceptors on bronchial smooth muscle, activating adenylyl cyclase and increasing cyclic AMP, which leads to smooth muscle relaxation and sustained bronchodilation.

DOSAGE AND ADMINISTRATION

•      1 capsule inhaled twice daily via dry powder inhaler

•      For inhalation use only — do not swallow

•      Rinse mouth with water after each dose to reduce risk of oral thrush

•      Assess asthma control regularly; step down when control is achieved

•      Use a reliever inhaler for acute symptoms

SIDE EFFECTS

Common:

•      Oral thrush, throat irritation

•      Headache, palpitations

•      Tremor

•      Hoarseness

Less common:

•      Adrenal suppression with prolonged high-dose use

•      Decreased bone density with long-term use

•      Hypokalemia at higher doses

•      Paradoxical bronchospasm

NOT RECOMMENDED IN

•      Active or quiescent pulmonary tuberculosis

•      Known hypersensitivity to budesonide, formoterol, or lactose

•      Not for use in acute asthma attacks

•      Not recommended in children under 12

DRUG INTERACTIONS

CYP3A4 inhibitors increase systemic budesonide exposure. Non-selective beta blockers antagonize formoterol. QT-prolonging drugs increase cardiovascular risk when used with formoterol.
