INDICATION

Clobederm-S Lotion 60ml is a combined superpotent corticosteroid and keratolytic lotion indicated for the treatment of hyperkeratotic and scaly inflammatory scalp conditions, including scalp psoriasis, seborrheic dermatitis with thick scaling, and lichen simplex chronicus of the scalp. The salicylic acid component removes thick scale, enhancing clobetasol penetration to the inflamed dermis for more complete therapeutic effect.

MECHANISM OF ACTION

Clobetasol propionate suppresses scalp inflammation through potent glucocorticoid receptor activation, reducing cytokine production and keratinocyte hyperproliferation. Salicylic acid is a beta-hydroxy acid keratolytic that softens and dissolves the intercellular bonds between corneocytes in the hyperkeratotic scale, facilitating removal of the scale layer and increasing the bioavailability and penetration of clobetasol to the inflamed scalp skin below. This synergistic combination provides faster scale clearance and more complete anti-inflammatory effect than either agent alone.

DOSAGE AND ADMINISTRATION

•      Apply to the scalp and affected areas once or twice daily as directed by the dermatologist

•      Part the hair to ensure contact of the lotion with the scalp skin

•      Maximum treatment duration: 2 weeks without reassessment

•      Do not exceed 50g per week; always follow your dermatologist's instructions

SIDE EFFECTS

Common:

•      Scalp burning and stinging (from salicylic acid and alcohol vehicle)

•      Scalp dryness and peeling (keratolytic effect)

•      Folliculitis

•      Hair thinning with prolonged use

Less common:

•      Salicylate toxicity with extensive application (tinnitus, dizziness; rare at standard scalp use)

•      HPA axis suppression

•      Secondary scalp infections

•      Contact sensitization to salicylic acid

DRUG INTERACTIONS

Salicylic acid enhances percutaneous absorption of clobetasol, increasing both efficacy and systemic absorption risk. Same systemic corticosteroid interactions as Clobederm Lotion with extensive application. Anticoagulants: monitor INR with extensive salicylate-containing applications.
