INDICATION

Clobederm Cream 20gm is a superpotent (Class I) topical corticosteroid indicated for the treatment of severe, resistant inflammatory skin conditions that do not respond adequately to less potent corticosteroid preparations. It is used for resistant eczema and atopic dermatitis, psoriasis (excluding widespread plaque psoriasis), lichen planus, lichen simplex chronicus, and other severe steroid-responsive dermatoses. The cream formulation is suitable for moist or weeping skin lesions where an ointment base is not preferred.

MECHANISM OF ACTION

Clobetasol propionate is a superpotent synthetic fluorinated corticosteroid that binds with very high affinity to intracellular glucocorticoid receptors, producing potent suppression of the inflammatory cascade. It reduces cytokine production (IL-1, IL-2, TNF-alpha), inhibits arachidonic acid metabolism, suppresses histamine and leukotriene release, and reduces vascular permeability and oedema in inflamed skin. The cream base is water-miscible and absorbs more readily than an ointment, making it suitable for inflamed, weeping, or exudative skin lesions where the occlusive ointment base is not tolerated.

DOSAGE AND ADMINISTRATION

•      Apply a thin layer to the affected area twice daily for up to 2 consecutive weeks only

•      Limit application to the smallest effective area to minimize systemic absorption

•      Do not use on the face, groin, axillae, or intertriginous areas unless specifically prescribed

•      Maximum weekly dose should not exceed 50g in adults; always follow your dermatologist's instructions

SIDE EFFECTS

Common:

•      Skin atrophy, thinning, and striae with prolonged or extensive use

•      Burning and stinging on application

•      Perilesional hypopigmentation

•      Folliculitis

Less common:

•      Clinically significant HPA axis suppression with extensive or prolonged use

•      Cushing's syndrome with excessive systemic absorption

•      Secondary bacterial or fungal skin infections

•      Glaucoma and cataracts with use near or on the eyes

NOT RECOMMENDED IN

•      Rosacea, acne vulgaris, and perioral dermatitis

•      Infected skin without adequate antimicrobial cover

•      Children under 12 years

•      Facial use, axillary or groin areas without dermatologist guidance

DRUG INTERACTIONS

Minimal systemic interactions at standard topical doses. Occlusive dressings significantly increase systemic absorption and risk of HPA suppression; use only under medical supervision. Widespread application can cause systemic corticosteroid drug interactions including with antidiabetics and immunosuppressants.
