INDICATION

Clindacure 300mg Cap is a lincosamide antibiotic used for the treatment of serious infections caused by susceptible anaerobic bacteria and certain Gram-positive organisms. Indications include pneumonia caused by aspiration, intra-abdominal infections, pelvic inflammatory disease, bacterial vaginosis, skin and soft tissue infections, osteomyelitis, septicemia, and as an alternative agent in patients with penicillin allergy.

MECHANISM OF ACTION

Clindamycin binds to the 50S ribosomal subunit of bacteria, inhibiting the peptidyl transferase step of protein synthesis. This bacteriostatic effect prevents bacterial growth. Clindamycin has excellent anaerobic coverage, including Bacteroides fragilis and other anaerobic cocci. It also demonstrates activity against certain Staphylococci and Streptococci. Its high tissue penetration, including into bone and abscess cavities, makes it useful in orthopedic and intra-abdominal infections.

DOSAGE AND ADMINISTRATION

•      Adults: 300mg to 450mg every 6 hours (up to 1800mg per day in severe infections)

•      Bacterial vaginosis: 300mg twice daily for 7 days

•      Aspiration pneumonia and intra-abdominal infections: 300mg to 450mg three to four times daily

•      Osteomyelitis: 300mg to 450mg every 6 hours for 4 to 6 weeks

•      Take with a full glass of water to prevent esophageal irritation

SIDE EFFECTS

Common:

•      Diarrhea (most common; can be severe)

•      Nausea, vomiting

•      Abdominal cramps

•      Rash

Less common:

•      Clostridium difficile-associated pseudomembranous colitis (potentially life-threatening; stop clindamycin immediately if suspected)

•      Elevated liver enzymes and jaundice

•      Esophageal ulceration if swallowed without adequate water

•      Anaphylaxis in hypersensitive patients

NOT RECOMMENDED IN

•      Known hypersensitivity to clindamycin or lincomycin

•      History of antibiotic-associated colitis or pseudomembranous colitis

•      Should not be used for meningitis (poor CNS penetration)

•      Use with caution in patients with inflammatory bowel disease

DRUG INTERACTIONS

Clindamycin may potentiate the effects of neuromuscular blocking agents such as suxamethonium and vecuronium. It antagonizes the effects of neostigmine and pyridostigmine used in myasthenia gravis. Clindamycin is a moderate CYP3A4 substrate and inhibitor. Concurrent use with erythromycin is antagonistic and should be avoided.
