INDICATION

Citanew Oral Drop 15ml is a selective serotonin reuptake inhibitor (SSRI) in oral drop form indicated for the treatment of major depressive disorder and generalized anxiety disorder in adults. The oral drop formulation allows flexible dose adjustment and is suitable for patients who have difficulty swallowing tablets, including the elderly and those requiring precise titration. It is also used in panic disorder, social anxiety disorder, and obsessive-compulsive disorder.

MECHANISM OF ACTION

Escitalopram is the pure S-enantiomer of citalopram and the most selective SSRI available. It inhibits the serotonin transporter (SERT) with high affinity and selectivity, blocking the reuptake of serotonin into presynaptic neurons and increasing serotonin availability in the synaptic cleft. As the active enantiomer of citalopram, escitalopram is approximately twice as potent with a cleaner receptor selectivity profile, producing fewer off-target effects than older SSRIs. Therapeutic effects typically emerge after 2 to 4 weeks of consistent use as downstream neuroplastic changes occur in mood-regulating brain circuits.

DOSAGE AND ADMINISTRATION

      Adults: 10mg (10ml) once daily in the morning or evening; may be increased to 20mg (20ml) once daily after at least 1 week if required

      Elderly patients: start at 5mg (5ml) once daily; maximum 10mg (10ml) per day

      Measure each dose carefully using the dropper; may be taken directly or mixed with water

SIDE EFFECTS

Common:

      Nausea (most common, usually transient in the first 1 to 2 weeks)

      Headache and dizziness

      Insomnia or somnolence

      Dry mouth and sweating

Less common:

      Sexual dysfunction: reduced libido, delayed orgasm, and erectile dysfunction

      QT prolongation at higher doses; monitor ECG in patients with cardiac risk factors

      Hyponatremia, particularly in elderly patients

      Serotonin syndrome when combined with other serotonergic drugs (rare)

NOT RECOMMENDED IN

      Concurrent use with MAO inhibitors (14-day washout required before and after escitalopram)

      Known hypersensitivity to escitalopram or citalopram

      QT prolongation or congenital long QT syndrome

      Concurrent use with pimozide or other QT-prolonging drugs without monitoring

DRUG INTERACTIONS

MAO inhibitors are contraindicated; a 14-day washout is required in both directions. Escitalopram prolongs the QT interval in a dose-dependent manner; avoid concurrent use with other QT-prolonging agents including antiarrhythmics, antipsychotics, and some antibiotics. Cimetidine inhibits CYP2C19 and CYP3A4, increasing escitalopram plasma levels; reduce dose if co-prescribed. Lithium and tryptophan increase serotonin syndrome risk; monitor closely. Anticoagulant effect of warfarin may be slightly enhanced; monitor INR. Pimozide is absolutely contraindicated due to QT prolongation risk.
