INDICATION

Ciplet 500mg Tab is a broad-spectrum fluoroquinolone antibiotic used for the treatment of respiratory tract infections, complicated urinary tract infections, pyelonephritis, gastrointestinal infections including typhoid fever, skin and soft tissue infections, bone and joint infections, intra-abdominal infections, and anthrax post-exposure prophylaxis.

MECHANISM OF ACTION

Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, disrupting DNA replication and transcription and causing bactericidal cell death. The 500mg dose achieves serum and tissue concentrations effective against more resistant or systemic infections. It demonstrates broad coverage against Gram-negative organisms including Pseudomonas aeruginosa and Enterobacteriaceae.

DOSAGE AND ADMINISTRATION

•      Respiratory tract infections: 500mg to 750mg twice daily for 7 to 14 days

•      Complicated UTI or pyelonephritis: 500mg twice daily for 7 to 14 days

•      Typhoid fever: 500mg twice daily for 10 to 14 days

•      Bone and joint infections: 500mg to 750mg twice daily for 4 to 6 weeks

•      Take with a full glass of water; avoid antacids and dairy within 2 hours of dosing

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhea

•      Headache, dizziness

•      Skin rash

•      Photosensitivity

Less common:

•      Tendon rupture, especially in patients over 60 or on corticosteroids

•      QT prolongation and ventricular arrhythmias

•      Seizures in patients with CNS disorders or renal impairment

•      Severe hypoglycemia when combined with antidiabetic drugs

NOT RECOMMENDED IN

•      Known hypersensitivity to ciprofloxacin or fluoroquinolones

•      History of tendon disorders following fluoroquinolone use

•      Concurrent use with tizanidine

•      Caution in patients with epilepsy, CNS disorders, or myasthenia gravis

DRUG INTERACTIONS

Same as Ciplet 250mg. At the 500mg dose, interactions with theophylline, warfarin, and NSAIDs require more careful monitoring. Ciprofloxacin should be used cautiously with drugs that prolong QT interval.
