INDICATION

Ciplet 250mg Tab is a fluoroquinolone antibiotic used to treat urinary tract infections (uncomplicated cystitis), gonorrhea, some gastrointestinal infections including salmonellosis and shigellosis, and as a component of anthrax post-exposure prophylaxis. The 250mg dose is primarily used for mild to moderate infections, particularly of the urinary tract and skin.

MECHANISM OF ACTION

Ciprofloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, enzymes essential for DNA replication, transcription, repair, and recombination. Inhibition of these enzymes leads to double-strand DNA breaks and rapid bactericidal death. It is active against a broad spectrum of Gram-negative bacteria.

DOSAGE AND ADMINISTRATION

•      Uncomplicated urinary tract infection: 250mg twice daily for 3 to 7 days

•      Uncomplicated gonorrhea: 500mg as a single dose

•      Take with plenty of water; maintain good hydration throughout treatment

•      Can be taken with or without food, but avoid dairy products and antacids around dose time

•      Complete the full prescribed course

SIDE EFFECTS

Common:

•      Nausea, diarrhoea, abdominal discomfort

•      Headache, dizziness

•      Rash

•      Elevated liver enzymes

Less common:

•      Tendinitis and tendon rupture, especially in elderly and patients on corticosteroids

•      QT prolongation and ventricular arrhythmias

•      Peripheral neuropathy

•      CNS effects including confusion, agitation, and seizures

NOT RECOMMENDED IN

•      Known hypersensitivity to ciprofloxacin or other fluoroquinolones

•      History of tendon disorders related to fluoroquinolone use

•      Use in children and adolescents (risk of cartilage damage; use only when no alternative)

•      Concurrent use with tizanidine

DRUG INTERACTIONS

Divalent and trivalent cations (antacids, iron, zinc, calcium) chelate ciprofloxacin and reduce its absorption by up to 90%; separate dosing by at least 2 to 4 hours. Ciprofloxacin inhibits CYP1A2 and increases plasma levels of theophylline, caffeine, clozapine, and ropinirole. It potentiates the anticoagulant effect of warfarin. Concurrent use with NSAIDs may increase the risk of seizures.
