INDICATION

Ciplet 250mg/5ml Susp is the higher-strength paediatric ciprofloxacin suspension for older children, adolescents, and adults unable to swallow tablets. It is used for the same indications as the 125mg/5ml formulation but provides higher doses suitable for older, heavier children and adults requiring liquid ciprofloxacin therapy.

MECHANISM OF ACTION

Same mechanism as Ciplet 125mg/5ml. Ciprofloxacin inhibits bacterial DNA gyrase and topoisomerase IV, causing bactericidal cell death through double-strand DNA breaks.

DOSAGE AND ADMINISTRATION

•      Older children and adolescents: 10 to 20mg/kg twice daily; maximum 750mg per dose, based on weight and indication

•      Adults unable to swallow tablets: 500mg equivalent (10ml) twice daily for most indications

•      Shake well before each dose

•      Always follow the weight-based dosage prescribed by your doctor; complete the full course

SIDE EFFECTS

Common:

•      Nausea, vomiting, and diarrhoea

•      Abdominal discomfort

•      Headache and dizziness

•      Rash

Less common:

•      Tendinitis and tendon rupture (monitor for tendon pain especially in older patients and corticosteroid users)

•      QT prolongation

•      Photosensitivity

•      Arthropathy in growing children

NOT RECOMMENDED IN

•      Known hypersensitivity to fluoroquinolones

•      Concurrent tizanidine

•      Avoid in routine paediatric infections where safer alternatives exist

•      Absorption reduced significantly by antacids, iron, calcium, and dairy products within 2 to 6 hours

DRUG INTERACTIONS

Antacids, iron, calcium, zinc reduce absorption. Tizanidine contraindicated. Warfarin: monitor INR. Theophylline levels increase. NSAIDs increase seizure risk.

STORAGE INSTRUCTIONS

Store dry powder below 25°C. After reconstitution: refrigerate and use within 14 days. Shake well before each use. Keep out of reach of children.
