INDICATION
Ciplet 125mg/5ml Syrup is a paediatric fluoroquinolone antibiotic suspension indicated for the treatment of bacterial infections in children caused by susceptible organisms. It is used for complicated urinary tract infections and pyelonephritis in children aged 1 to 17 years, inhalational anthrax (post-exposure prophylaxis and treatment), and as an alternative when first-line antibiotics are contraindicated or ineffective. Ciprofloxacin is not a first-line antibiotic for children due to potential concerns about cartilage development; use is restricted to specific indications.
MECHANISM OF ACTION
Ciprofloxacin is a broad-spectrum fluoroquinolone antibiotic that inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV, two essential bacterial enzymes required for DNA replication, transcription, repair, and recombination. Inhibition of these enzymes causes breaks in the bacterial DNA, preventing bacterial cell division and ultimately causing bactericidal cell death. Ciprofloxacin has excellent activity against most gram-negative aerobes.
DOSAGE AND ADMINISTRATION
• Children (UTI): 10 to 20mg/kg twice daily; maximum 750mg per dose
• Inhalational anthrax: 15mg/kg twice daily; maximum 500mg per dose for 60 days
• Shake well before each dose; the 125mg/5ml suspension is specifically designed for paediatric weight-based dosing
• Always follow the weight-based dosage and course duration prescribed by your pediatrician; not for routine first-line use in children
SIDE EFFECTS
Common:
• Nausea, vomiting, and diarrhoea
• Abdominal pain
• Headache and dizziness
• Skin rash
Less common:
• Arthropathy and joint pain (theoretical concern in weight-bearing joints in growing children; monitor)
• Tendinitis and tendon rupture (risk increased with corticosteroids; monitor for tendon pain)
• QT prolongation (rare)
• Photosensitivity (advise sun protection)
NOT RECOMMENDED IN
• Not for routine use as a first-line antibiotic in children due to theoretical cartilage toxicity concerns
• Known hypersensitivity to ciprofloxacin or other fluoroquinolones
• Concurrent use with tizanidine (markedly increases tizanidine levels)
• Concurrent antacids, iron, calcium, or zinc within 2 to 6 hours (reduce absorption significantly)
DRUG INTERACTIONS
Antacids, iron, calcium, zinc, and sucralfate reduce ciprofloxacin absorption by 40 to 90%; take ciprofloxacin 2 hours before or 6 hours after these products. Tizanidine is contraindicated with ciprofloxacin. Warfarin effect is significantly enhanced; monitor INR. Theophylline levels are increased by ciprofloxacin (CYP1A2 inhibition); reduce theophylline dose and monitor levels. NSAIDs combined with fluoroquinolones increase seizure risk.
STORAGE INSTRUCTIONS
Before reconstitution: store below 25°C. After reconstitution: refrigerate and use within 14 days. Shake well before each use. Keep out of reach of children.
