INDICATION
Cilapen 500 IV Inj is a carbapenem-class antibiotic combination indicated for the treatment of serious and life-threatening bacterial infections caused by susceptible organisms in adults and children aged 1 year and above. It is used for severe hospital-acquired and ventilator-associated pneumonia, complicated intra-abdominal infections, complicated urinary tract infections, skin and soft tissue infections, septicemia, and febrile neutropenia. It is reserved for infections caused by multidrug-resistant gram-negative organisms and polymicrobial infections requiring broad-spectrum coverage.
MECHANISM OF ACTION
Imipenem is a carbapenem beta-lactam antibiotic with the broadest spectrum of antibacterial activity of all beta-lactam antibiotics. It inhibits bacterial cell wall synthesis by binding with high affinity to penicillin-binding proteins, blocking peptidoglycan cross-linking and causing bacterial cell death. It is stable to most bacterial beta-lactamases, including extended-spectrum beta-lactamases and AmpC enzymes, providing activity against many resistant gram-negative pathogens. Cilastatin is a specific reversible inhibitor of dehydropeptidase-I, the renal tubular enzyme that metabolizes and inactivates imipenem in the kidney. By blocking this enzyme, Cilastatin prevents imipenem degradation, extends its antibacterial activity, and prevents renal accumulation of potentially nephrotoxic metabolites.
DOSAGE AND ADMINISTRATION
• Adults: 500mg/500mg by IV infusion every 6 to 8 hours; may increase to 1g/1g every 6 hours for severe infections
• Administer each dose over 20 to 30 minutes (for doses up to 500mg) or 40 to 60 minutes (for doses above 500mg)
• Dose reduction required based on creatinine clearance in renal impairment
• Always administered by trained healthcare professionals in a hospital or clinical setting
SIDE EFFECTS
Common:
• Nausea and vomiting during infusion (slow the infusion rate if nausea occurs)
• Diarrhoea
• Thrombophlebitis at the infusion site
• Elevated liver enzymes
Less common:
• Seizures (risk increased at high doses, in renal impairment, or in patients with pre-existing CNS disorders; reduce dose and monitor)
• Clostridium difficile-associated diarrhoea
• Hypersensitivity reactions including anaphylaxis
• Myelosuppression with prolonged high-dose therapy
NOT RECOMMENDED IN
• Known hypersensitivity to imipenem, Cilastatin, or other carbapenems and beta-lactam antibiotics
• Do not combine with ertapenem or other carbapenems without specific indication
• Administer cautiously in patients with a history of seizures or CNS disorders; reduce dose in renal impairment
• Not for intramuscular administration in this formulation
DRUG INTERACTIONS
Ganciclovir combined with imipenem-Cilastatin significantly increases seizure risk; avoid combination. Valproic acid plasma levels are dramatically reduced by carbapenem antibiotics due to enhanced glucuronidation; the combination may precipitate seizures in epileptic patients on valproate; consider alternative antibiotic or alternative anticonvulsant. Probenecid increases imipenem plasma levels by inhibiting renal tubular secretion.
