INDICATION

Cidpro 40mg Cap is a higher-dose proton pump inhibitor prescribed for severe or erosive gastroesophageal reflux disease, Zollinger-Ellison syndrome, large or refractory peptic ulcers, and as part of H. pylori eradication therapy. The 40mg dose provides more complete acid suppression than the 20mg dose, making it appropriate for patients with more significant pathology or inadequate response to lower doses.

MECHANISM OF ACTION

Omeprazole is irreversibly activated by gastric acid to a sulphenamide form, which covalently binds to the cysteine residues of the H+/K+-ATPase proton pump in the parietal cell. At 40mg, omeprazole produces near-complete suppression of acid secretion over 24 hours, creating an optimal environment for mucosal healing and for antibiotic-mediated H. pylori eradication.

DOSAGE AND ADMINISTRATION

•      Erosive esophagitis and severe GERD: 40mg once daily for 4 to 8 weeks

•      Zollinger-Ellison syndrome: 60mg to 120mg daily in divided doses

•      H. pylori eradication: 40mg once daily or 20mg twice daily with antibiotics

•      Refractory peptic ulcer: 40mg once daily for 8 weeks

•      Take 30 to 60 minutes before the first meal

SIDE EFFECTS

Common:

•      Headache, nausea, diarrhea

•      Abdominal discomfort, flatulence

•      Dry mouth

•      Dizziness

Less common:

•      Hypomagnesaemia with chronic use

•      Vitamin B12 malabsorption with long-term use

•      Risk of bone fractures with prolonged use at high doses

•      Clostridium difficile infection

NOT RECOMMENDED IN

•      Known hypersensitivity to omeprazole or any PPI

•      Concurrent use with nelfinavir

•      Avoid prolonged use at high doses without medical review

DRUG INTERACTIONS

At higher doses, inhibition of CYP2C19 is more pronounced, potentially increasing the interaction magnitude with methotrexate, clopidogrel, warfarin, and phenytoin. Monitor INR in patients on warfarin.
