INDICATION

Cidpro 20mg Cap is a proton pump inhibitor (PPI) used to treat gastroesophageal reflux disease (GERD), peptic ulcer disease (gastric and duodenal ulcers), and Helicobacter pylori infection as part of combination antibiotic therapy. It is also used for the treatment of Zollinger-Ellison syndrome and prevention of NSAID-induced gastric ulcers in high-risk patients.

MECHANISM OF ACTION

Omeprazole is a prodrug that requires activation in the acidic environment of the gastric parietal cell's secretory canaliculi. Once activated, it covalently binds to and irreversibly inhibits the hydrogen-potassium ATPase (H+/K+-ATPase) enzyme, the final step in gastric acid secretion. This dramatically reduces both basal and stimulated gastric acid production, raising intragastric pH and allowing ulcers and inflamed mucosa to heal.

DOSAGE AND ADMINISTRATION

•      GERD: 20mg once daily for 4 weeks; may extend to 8 weeks if incomplete healing

•      Duodenal ulcer: 20mg once daily for 2 to 4 weeks

•      Gastric ulcer: 20mg once daily for 4 to 8 weeks

•      H. pylori eradication: 20mg twice daily with antibiotics for 7 to 14 days

•      Take 30 to 60 minutes before the first meal of the day

SIDE EFFECTS

Common:

•      Headache, nausea

•      Diarrhea, abdominal pain

•      Constipation, flatulence

•      Dizziness

Less common:

•      Hypomagnesaemia with long-term use

•      Vitamin B12 deficiency

•      Clostridium difficile-associated diarrhea

•      Bone fracture risk with prolonged high-dose use

NOT RECOMMENDED IN

•      Known hypersensitivity to omeprazole or other PPIs

•      Concurrent use with nelfinavir

•      Use with caution in patients with severe liver disease

DRUG INTERACTIONS

Omeprazole is a moderate CYP2C19 inhibitor and can increase plasma levels of clopidogrel's active metabolite by reducing its conversion, potentially reducing antiplatelet effect. It increases the plasma levels of diazepam, warfarin, and phenytoin. It reduces the absorption of drugs requiring acidic pH, including ketoconazole, itraconazole, and atazanavir.
