INDICATION

Cefot 2gm Inj is the higher-strength formulation of cefoperazone-sulbactam indicated for the treatment of severe bacterial infections including hospital-acquired and ventilator-associated pneumonia, severe intra-abdominal sepsis, complicated urinary tract infections, septicemia, and multidrug-resistant gram-negative infections where higher cefoperazone doses are required. The 2g vial provides a full adult therapeutic dose in a single vial.

MECHANISM OF ACTION

Same mechanism as Cefot 1gm. The 2g vial (1g cefoperazone + 1g sulbactam) provides higher peak plasma concentrations and greater antibacterial efficacy against organisms with higher minimum inhibitory concentrations. In very severe infections with Pseudomonas aeruginosa and ESBL-producing gram-negative bacteria, higher doses increase the probability of pharmacodynamic target attainment.

DOSAGE AND ADMINISTRATION

•      Adults: 2g (1 vial) every 12 hours by slow IV infusion for moderate to severe infections

•      Severe or refractory infections: up to 4g per dose every 12 hours under specialist guidance

•      Infuse over 15 to 60 minutes; dilute in normal saline or 5% dextrose

•      Prescribed and administered by trained healthcare professionals; avoid alcohol during and for 5 days after treatment

SIDE EFFECTS

Common:

•      Diarrhoea and Clostridium difficile risk (higher with 2g doses)

•      Rash and urticaria

•      Elevated liver enzymes

•      Eosinophilia

Less common:

•      Hypoprothrombinemia and bleeding (more significant risk at higher doses; monitor PT/INR)

•      Hypersensitivity reactions

•      Neurotoxicity (rare; seizures at very high doses in renal impairment)

NOT RECOMMENDED IN

•      Known hypersensitivity to cephalosporins or sulbactam

•      Uncorrected bleeding disorders

•      Severe hepatic impairment without dose adjustment

DRUG INTERACTIONS

Warfarin effect is significantly enhanced; monitor PT/INR. Alcohol: disulfiram-like reaction; strictly avoid. Vitamin K supplementation may be required during prolonged courses.
