INDICATION
Cefot 1gm Inj is a combined third-generation cephalosporin and beta-lactamase inhibitor antibiotic indicated for the treatment of moderate to severe bacterial infections caused by susceptible organisms, including intra-abdominal infections, gynecological infections, lower respiratory tract infections including hospital-acquired pneumonia, urinary tract infections, septicemia, and skin and soft tissue infections. The sulbactam component extends the spectrum to include beta-lactamase-producing organisms.
MECHANISM OF ACTION
Cefoperazone is a third-generation cephalosporin antibiotic effective against many gram-negative bacteria, including Pseudomonas aeruginosa. It works by inhibiting bacterial cell wall synthesis through binding to penicillin-binding proteins. Sulbactam is a beta-lactamase inhibitor that protects cefoperazone from bacterial enzymes that cause resistance. Together, they provide broad-spectrum activity against gram-negative, anaerobic, and some gram-positive bacteria, including ESBL-producing organisms.
DOSAGE AND ADMINISTRATION
• Adults: 2 to 4g per day in two divided doses by IV infusion or IM injection; may be increased to 8g/day in severe infections
• 1g vial contains 500mg cefoperazone + 500mg sulbactam; standard dose is 1 to 2 vials (1 to 2g) every 12 hours
• Administer by slow IV infusion over 15 to 60 minutes or by deep IM injection
• Prescribed and administered by trained healthcare professionals; dose adjustment required in hepatic impairment
SIDE EFFECTS
Common:
• Diarrhoea (most common, including risk of antibiotic-associated diarrhoea)
• Skin rash and urticaria
• Elevated liver enzymes and hyperbilirubinemia
• Eosinophilia
Less common:
• Hypoprothrombinemia and bleeding
• Hypersensitivity reactions including anaphylaxis
NOT RECOMMENDED IN
• Known hypersensitivity to cephalosporins, penicillins, or sulbactam
• Patients with bleeding disorders or on anticoagulant therapy (additional monitoring required due to hypoprothrombinemia risk)
• Severe hepatic impairment (dose adjustment and monitoring required)
DRUG INTERACTIONS
Anticoagulants such as warfarin have significantly enhanced effects when combined with cefoperazone due to hypoprothrombinemia; monitor PT/INR and adjust anticoagulant dose. Alcohol causes a disulfiram-like reaction (flushing, nausea, palpitations, hypotension); strictly avoid during treatment and for 5 days after. Vitamin K supplementation may be required during prolonged courses, particularly in malnourished patients.
