INDICATION

Carpi 3mg Cap is used for the treatment of schizophrenia, acute manic or mixed episodes in bipolar I disorder, bipolar depression, and as adjunctive therapy for major depressive disorder in adults. The 3mg dose is the most commonly used maintenance dose across all approved indications and is the dose to which most patients are titrated after starting at 1.5mg.

MECHANISM OF ACTION

Same as Carpi 1.5mg. At 3mg once daily, cariprazine achieves clinical D3 and D2 partial agonism sufficient for antipsychotic and mood-stabilizing effects. The 3mg dose provides the optimal balance between therapeutic efficacy and tolerability for most patients with schizophrenia and bipolar I disorder. In bipolar depression, the 3mg dose (after 2 weeks on 1.5mg) provides meaningful antidepressant benefit through D3/5-HT1A partial agonism without the akathisia burden sometimes seen at higher doses.

DOSAGE AND ADMINISTRATION

•      Schizophrenia: 3mg to 6mg once daily (3mg is the most common effective dose)

•      Bipolar I mania: 3mg to 6mg once daily

•      Bipolar depression: increase to 3mg after 2 weeks on 1.5mg if well tolerated

•      MDD (adjunctive): 1.5mg to 3mg once daily

•      Take with or without food at the same time each day

•      When co-administered with strong CYP3A4 inhibitors: reduce dose by 50%

SIDE EFFECTS

Common:

•      Akathisia and extrapyramidal symptoms (more frequent at higher doses)

•      Somnolence, dizziness, fatigue

•      Nausea, constipation, dyspepsia, vomiting

•      Weight gain

Less common:

•      Tardive dyskinesia with prolonged use

•      Neuroleptic malignant syndrome (rare)

•      Hyperglycemia and dyslipidemia

•      Orthostatic hypotension

•      Leucopenia and agranulocytosis (rare)

NOT RECOMMENDED IN

•      Known hypersensitivity to cariprazine

•      Dementia-related psychosis in elderly patients

•      Children and adolescents below 18 years

•      Pregnancy — use only if clearly necessary

•      Caution in patients with seizure history or cardiovascular disease

DRUG INTERACTIONS

Same as Carpi 1.5mg. At 3mg, the interaction with strong CYP3A4 inhibitors is particularly important — the dose should be halved to 1.5mg when combined with ketoconazole, clarithromycin, or ritonavir. Avoid strong CYP3A4 inducers as they substantially reduce cariprazine efficacy. Additive CNS depression with alcohol and sedatives.
