INDICATION

Carpi 1.5mg Cap is an atypical antipsychotic used for the treatment of schizophrenia in adults, acute manic or mixed episodes associated with bipolar I disorder in adults, depressive episodes associated with bipolar I disorder (bipolar depression), and as adjunctive therapy to antidepressants in major depressive disorder (MDD). The 1.5mg dose is the standard starting dose for most indications.

MECHANISM OF ACTION

Cariprazine is a partial agonist at dopamine D3 and D2 receptors, with preferential affinity for D3 receptors, and a partial agonist at serotonin 5-HT1A receptors. It also acts as an antagonist at 5-HT2A and 5-HT2B receptors. Unlike many older antipsychotics that are full D2 antagonists, cariprazine's partial agonism at D2 and D3 receptors allows it to both reduce excessive dopamine activity (in conditions of hyperdopaminergic, such as psychosis) and supplement insufficient dopamine activity (in conditions of hypodopaminergic, such as depression and negative symptoms). The preferential D3 activity is thought to improve cognitive function and negative symptoms of schizophrenia, which are not adequately addressed by most other antipsychotics. Cariprazine and its two active metabolites (DCAR and DDCAR) have long half-lives allowing once-daily dosing with effects persisting for weeks after discontinuation.

DOSAGE AND ADMINISTRATION

      Schizophrenia: start at 1.5mg once daily on Day 1; may increase to 3mg on Day 2; recommended range 1.5mg to 6mg once daily

      Bipolar I mania: start at 1.5mg once daily; may increase to 3mg on Day 2; recommended range 3mg to 6mg daily

      Bipolar depression: 1.5mg once daily; may increase to 3mg after 2 weeks if needed

      MDD (adjunctive): 1.5mg once daily; may increase to 3mg after 2 weeks

      Take with or without food; take at the same time each day

      Do not stop abruptly; taper if discontinuing

      Dose adjustment required with strong CYP3A4 inhibitors or inducers

SIDE EFFECTS

Common:

      Extrapyramidal symptoms (EPS): akathisia, tremor, rigidity, parkinsonism

      Somnolence, dizziness, fatigue

      Nausea, constipation, dyspepsia

      Weight gain and metabolic changes

Less common:

      Tardive dyskinesia with long-term use — may be irreversible

      Neuroleptic malignant syndrome (NMS) — rare but life-threatening

      Hyperglycemia and diabetes mellitus

      Orthostatic hypotension and syncope

      Leucopenia and neutropenia

      Potential for lenticular changes or cataracts — ophthalmological monitoring recommended

NOT RECOMMENDED IN

      Known hypersensitivity to cariprazine or any excipient

      Not approved for dementia-related psychosis in elderly patients — increased risk of mortality

      Caution in patients with cardiovascular or cerebrovascular disease

      Caution in patients with a history of seizures

      Not established for use in children and adolescents below 18 years

      Pregnancy — use only if benefit outweighs risk; potential neonatal withdrawal symptoms

DRUG INTERACTIONS

Cariprazine is primarily metabolized by CYP3A4. Strong CYP3A4 inhibitors (ketoconazole, clarithromycin, itraconazole, ritonavir) significantly increase cariprazine and active metabolite plasma concentrations; reduce cariprazine dose by half when used concomitantly. Strong CYP3A4 inducers (rifampicin, carbamazepine, phenytoin, St. John's Wort) reduce cariprazine exposure substantially; the combination should be avoided. Cariprazine has additive CNS depressant effects with alcohol, sedatives, opioids, and benzodiazepines. Unlike many antipsychotics, cariprazine does not significantly prolong the QT interval, reducing the risk of arrhythmia interactions.

STORAGE INSTRUCTIONS

Store Carpi 1.5mg capsules at room temperature between 20°C and 25°C. Protect from light. Keep in original blister packaging. Store out of reach of children.
