INDICATION

Cardiolite 100mg Tab is a cardioselective beta-1 adrenergic blocker indicated for the treatment of hypertension, stable angina pectoris, and management of cardiac arrhythmias including supraventricular tachycardia. It is also indicated for long-term management following acute myocardial infarction to reduce mortality, and for the prevention of migraine headaches. The 100-tablet pack is suitable for patients on long-term chronic therapy.

MECHANISM OF ACTION

Atenolol selectively blocks beta-1 adrenergic receptors in the heart, reducing heart rate, myocardial contractility, and cardiac output. These effects collectively lower blood pressure, reduce myocardial oxygen demand, and control tachyarrhythmias by slowing conduction through the atrioventricular node. Its cardioselectivity for beta-1 over beta-2 receptors minimizes adverse effects on bronchial smooth muscle and peripheral vasculature, making it safer than non-selective beta-blockers in patients with mild to moderate obstructive airways disease. Atenolol is hydrophilic, resulting in minimal CNS penetration and reduced incidence of nightmares and depression compared to lipophilic beta-blockers.

DOSAGE AND ADMINISTRATION

      Hypertension and angina: 50 to 100mg once daily

      Higher doses up to 200mg may be used in arrhythmia management under cardiologist guidance

      Take at the same time each day with or without food

      Never stop abruptly; taper the dose gradually over 1 to 2 weeks under medical supervision

SIDE EFFECTS

Common:

      Bradycardia (slow heart rate)

      Fatigue and weakness

      Cold extremities (peripheral vasoconstriction)

      Dizziness and lightheadedness

Less common:

      Bronchospasm in asthmatic patients (use with caution; non-selective bronchospasm risk even with beta-1 selective agents)

      Erectile dysfunction and reduced libido

      Depression (less common than with lipophilic beta-blockers)

      AV block in susceptible patients

NOT RECOMMENDED IN

      Cardiogenic shock or decompensated heart failure requiring inotropic support

      Second or third-degree AV block without a functioning pacemaker

      Severe bradycardia (below 50 beats per minute) or sick sinus syndrome

      Known hypersensitivity to atenolol; severe or uncontrolled asthma

DRUG INTERACTIONS

Verapamil and diltiazem combined with atenolol significantly increase the risk of AV block, bradycardia, and cardiac arrest; avoid combination particularly with intravenous verapamil. Antihypertensives have additive blood pressure-lowering effects. Clonidine: taper atenolol before stopping clonidine to prevent rebound hypertension. NSAIDs reduce antihypertensive efficacy. Beta-blockers mask the Hypoglycaemia tachycardia warning signal in diabetic patients on insulin.
