INDICATION

Carbokyt 500mg/10ml Inj is a high-dose intravenous iron preparation indicated for the treatment of iron-deficiency anemia when oral iron therapy is insufficient, not tolerated, or not feasible. It is used in patients with chronic kidney disease, heart failure, inflammatory bowel disease, post-partum iron deficiency, and severe iron deficiency requiring rapid iron repletion. It can deliver up to 1000mg of elemental iron in a single infusion session, providing more efficient iron repletion than older intravenous iron preparations.

MECHANISM OF ACTION

Ferric carboxymaltose is an iron-carbohydrate complex where polynuclear iron (III)-hydroxide is stabilized by a carboxymaltose shell, allowing large doses of iron to be administered safely in a short infusion. After intravenous administration, the complex is taken up by the reticuloendothelial system through endocytosis. The carboxymaltose shell is gradually metabolized, releasing iron that binds to transferrin and is transported to erythroid precursors for hemoglobin synthesis and to ferritin for storage. The controlled release prevents free iron-mediated oxidative toxicity and anaphylaxis.

DOSAGE AND ADMINISTRATION

•      Adults: up to 1000mg elemental iron (2 vials of 500mg) by slow IV infusion once weekly until total iron deficit is corrected

•      Maximum single dose: 1000mg iron per infusion session

•      Administer by slow IV infusion diluted in normal saline over at least 15 minutes; never administer as an undiluted IV bolus

•      All doses calculated using the Ganzoni formula based on body weight and hemoglobin; prescribed and administered by healthcare professionals only

SIDE EFFECTS

Common:

•      Hypophosphatasemia with repeated dosing (monitor serum phosphate)

•      Nausea, vomiting, and abdominal discomfort

•      Headache and dizziness

•      Transient hypotension with rapid infusion (infuse slowly)

Less common:

•      Hypersensitivity reactions including anaphylactoid reactions (rare but serious; have resuscitation equipment available)

•      Injection site skin discoloration and pruritus if extravasation occurs (avoid subcutaneous administration)

•      Arthralgia and myalgia

•      Iron overload with excessive cumulative dosing

NOT RECOMMENDED IN

•      Iron overload conditions (haemochromatosis)

•      Anemia not caused by iron deficiency

•      Known hypersensitivity to ferric carboxymaltose or any excipient

•      First trimester of pregnancy without clear medical necessity

DRUG INTERACTIONS

Oral iron should not be initiated within 5 days of intravenous ferric carboxymaltose administration as transferrin binding sites may be saturated. ACE inhibitors may increase the risk of systemic reactions with intravenous iron; monitor closely. Do not mix with other medications or solutions other than normal saline without confirmed compatibility data.
