INDICATION

Canter 500mg Tab is a macrolide antibiotic indicated for the treatment of mild to moderate bacterial infections caused by susceptible organisms including community-acquired pneumonia, acute exacerbations of chronic bronchitis, pharyngitis and tonsillitis, sinusitis, skin and soft tissue infections, and Helicobacter pylori eradication as part of triple therapy regimens. It is also used for the treatment and prophylaxis of Mycobacterium avium complex (MAC) infections in HIV-positive patients.

MECHANISM OF ACTION

Clarithromycin is a 14-membered macrolide antibiotic derived from erythromycin with improved acid stability and oral bioavailability. It inhibits bacterial protein synthesis by reversibly binding to the 50S ribosomal subunit and blocking the translocation step of peptide chain elongation, producing bacteriostatic effects at standard concentrations and bactericidal activity against some organisms at higher concentrations. Clarithromycin also has an active metabolite, 14-hydroxyclarithromycin, which contributes to its antimicrobial activity. It has excellent tissue penetration and achieves high intracellular concentrations in macrophages and polymorphonuclear cells.

DOSAGE AND ADMINISTRATION

•      Adults: 250 to 500mg twice daily for 7 to 14 days depending on indication

•      H. pylori eradication: 500mg twice daily with amoxicillin and a proton pump inhibitor for 7 to 14 days

•      Community-acquired pneumonia: 500mg twice daily for 7 to 14 days

•      Always complete the full prescribed course as directed by your doctor

SIDE EFFECTS

Common:

•      Nausea, vomiting, diarrhoea, and abdominal pain (most common)

•      Taste perversion (metallic or bitter taste; very characteristic of clarithromycin)

•      Headache

•      Elevated liver enzymes

Less common:

•      QT prolongation and cardiac arrhythmias in susceptible patients

•      Clostridium difficile-associated diarrhoea

•      Hepatotoxicity with elevated bilirubin and jaundice (rare)

•      Hypersensitivity reactions including rash, urticaria, and anaphylaxis

NOT RECOMMENDED IN

•      Known hypersensitivity to clarithromycin, erythromycin, or other macrolide antibiotics

•      Concurrent use with drugs contraindicated due to QT prolongation or CYP3A4 inhibition (pimozide, cisapride, terfenadine)

•      Severe hepatic impairment

•      Pregnancy (particularly in the first trimester; limited safety data)

DRUG INTERACTIONS

Clarithromycin is a potent CYP3A4 inhibitor and significantly increases plasma levels of many co-administered drugs including statins (rhabdomyolysis risk; avoid simvastatin and lovastatin), ciclosporin, tacrolimus, digoxin, warfarin, and colchicine. Rifampicin dramatically reduces clarithromycin levels through CYP3A4 induction; avoid combination. QT-prolonging drugs should not be co-administered. Antacids slightly reduce clarithromycin absorption rate but not overall bioavailability.
