INDICATION

Calcium Gluconate Injection is used for the emergency treatment of severe hypocalcemia (low blood calcium), hyperkalemia-induced cardiac arrhythmias, hypermagnesemia, and as an antidote to calcium channel blocker toxicity and hydrofluoric acid burns. It is also given as a supplement in neonates, in patients with hungry bone syndrome post-parathyroid surgery, and to counteract the cardiac effects of severe electrolyte disturbances.

MECHANISM OF ACTION

Calcium gluconate delivers ionized calcium to restore physiological calcium levels. Calcium is essential for numerous cellular processes including cardiac and skeletal muscle contraction, nerve impulse transmission, blood coagulation, and enzyme activation. In hyperkalemia, IV calcium acts rapidly to stabilize the cardiac cell membrane by raising the threshold potential, counteracting the depolarizing effects of excess potassium on the myocardium and reducing the risk of ventricular fibrillation.

DOSAGE AND ADMINISTRATION

      Hypocalcemia (acute): 10ml of 10% solution IV over 10 minutes, repeated as needed based on response

      Hyperkalemia with cardiac effects: 10ml to 20ml of 10% solution IV over 5 to 10 minutes under ECG monitoring

      Neonatal hypocalcemia: 1ml/kg IV slowly under supervision

      Calcium channel blocker toxicity: 20ml to 60ml IV bolus as initial treatment

      Infuse slowly; rapid IV injection can cause bradycardia and cardiac arrest

SIDE EFFECTS

Common:

      Sensation of warmth or flushing during infusion

      Mild hypotension during rapid injection

      Nausea and vomiting

      Tingling or paresthesia

Less common:

      Severe bradycardia or cardiac arrest with rapid IV push

      Hypercalcinemia with excessive or repeated dosing

      Tissue necrosis from extravasation

      Nephrolithiasis (kidney stones) with prolonged therapy

NOT RECOMMENDED IN

      hypercalcemia or hypercalciuria

      Ventricular fibrillation during cardiac resuscitation

      Concurrent use with digitalis glycosides (risk of fatal arrhythmia)

      Do not mix with sodium bicarbonate or phosphate solutions (precipitation occurs)

DRUG INTERACTIONS

Calcium gluconate is physically incompatible with many medications and should not be mixed in IV lines with sodium bicarbonate, phosphate solutions, ceftriaxone, or fluoroquinolones. Concurrent use with digoxin or other cardiac glycosides increases the risk of digitalis toxicity and fatal arrhythmia; avoid unless clearly indicated. Thiazide diuretics reduce urinary calcium excretion and can worsen hypercalcemia if calcium gluconate is given simultaneously.
