INDICATION
Calamox Drops 20ml is a Paediatric co-amoxiclav (amoxicillin-clavulanate) suspension indicated for the treatment of mild to moderate bacterial infections in infants and young children caused by beta-lactamase-producing organisms that are resistant to amoxicillin alone. It is used for otitis media, sinusitis, lower respiratory tract infections including community-acquired pneumonia, urinary tract infections, skin and soft tissue infections, and dental infections where susceptible gram-positive and gram-negative organisms, including beta-lactamase producers, are involved.
MECHANISM OF ACTION
Amoxicillin is a broad-spectrum aminopenicillin that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins and blocking peptidoglycan cross-linking, causing bactericidal death. Clavulanic acid is an irreversible beta-lactamase inhibitor that covalently binds to and permanently inactivates the beta-lactamase enzymes produced by resistant bacteria such as Staphylococcus aureus, Haemophilus influenzae, Moraxella catarrhalis, Klebsiella, and Escherichia coli. By protecting amoxicillin from enzymatic degradation, clavulanic acid restores its antibacterial activity against beta-lactamase-producing organisms, significantly extending the spectrum compared to amoxicillin alone. The Paediatric drops formulation provides a concentrated, measurable dose for accurate weight-based dosing in infants.
DOSAGE AND ADMINISTRATION
• Infants and children under 3 months: 30mg/kg/day of the amoxicillin component in divided doses every 12 hours
• Children 3 months to 12 years: 25 to 45mg/kg/day of the amoxicillin component in divided doses every 8 to 12 hours based on severity
• Shake the drops well before measuring each dose; may be mixed with milk or formula
• Always follow the weight-based dosage and course duration prescribed by your pediatrician; complete the full prescribed course even if symptoms improve early
SIDE EFFECTS
Common:
• Diarrhoea (most common; more frequent with co-amoxiclav than amoxicillin alone due to clavulanic acid)
• Nausea and vomiting
• Skin rash (assess whether allergic or non-allergic if rash appears)
• Abdominal discomfort and bloating
Less common:
• Hepatotoxicity: elevated liver enzymes and cholestatic jaundice (more associated with co-amoxiclav than amoxicillin alone; monitor LFTs if symptoms arise)
• Allergic reactions including urticaria, angioedema, and anaphylaxis in penicillin-hypersensitive patients
• Clostridium difficile-associated diarrhoea with prolonged use

NOT RECOMMENDED IN
• Known hypersensitivity to penicillins, cephalosporins, or other beta-lactam antibiotics
• History of co-amoxiclav-associated jaundice or hepatic dysfunction
• Infectious mononucleosis (widespread non-allergic rash; amoxicillin contraindicated in this setting)
• Dose adjustment required in renal impairment; not effective against MRSA or organisms producing extended-spectrum beta-lactamases (ESBLs)
DRUG INTERACTIONS
Methotrexate renal elimination is reduced by amoxicillin component, increasing methotrexate toxicity risk; monitor closely. Warfarin anticoagulant effect may be enhanced; monitor INR. Probenecid reduces renal tubular secretion of amoxicillin, prolonging plasma half-life. Allopurinol combined with amoxicillin increases skin rash incidence. Oral contraceptives may theoretically have reduced efficacy during antibiotic therapy; advise additional contraceptive precautions if clinically relevant.
