INDICATION

Butrin 75mg Tab is a norepinephrine-dopamine reuptake inhibitor (NDRI) used for the treatment of major depressive disorder and seasonal affective disorder. It is also used as an aid for smoking cessation. Unlike many other antidepressants, bupropion does not significantly affect serotonin or cause sexual dysfunction, and tends to promote weight loss rather than weight gain.

MECHANISM OF ACTION

Bupropion inhibits the neuronal reuptake of norepinephrine and dopamine by blocking their respective transporters in the central nervous system. This increases the availability of these neurotransmitters in the synaptic cleft, enhancing dopaminergic and noradrenergic neurotransmission, which contributes to improved mood, motivation, and energy. Bupropion also has mild antagonistic activity at nicotinic acetylcholine receptors, which underlies its efficacy in reducing nicotine cravings.

DOSAGE AND ADMINISTRATION

•      Depression: starting dose 75mg twice daily; may increase to 150mg twice daily after at least 3 days

•      Maximum dose: 450mg per day in divided doses; no single dose above 150mg

•      Take with food to reduce nausea and insomnia risk

•      Swallow tablets whole — do not crush or chew

•      Allow at least 6 hours between doses; avoid taking close to bedtime

SIDE EFFECTS

Common:

•      Dry mouth, headache

•      Nausea, constipation

•      Insomnia or sleep disturbance

•      Increased sweating, tremor

Less common:

•      Seizures, especially at doses above 450mg/day or in predisposed patients

•      Hypertension; blood pressure should be monitored

•      Psychiatric symptoms including agitation and mania in bipolar patients

•      Rare: Stevens-Johnson syndrome or anaphylactic reaction

NOT RECOMMENDED IN

•      History of seizure disorders or conditions that lower seizure threshold

•      Current or prior diagnosis of anorexia nervosa or bulimia nervosa

•      Concurrent use with MAOIs (14-day washout required)

•      Abrupt discontinuation of alcohol or sedatives

•      Known hypersensitivity to bupropion

DRUG INTERACTIONS

Bupropion is metabolised by CYP2B6 and is itself a potent CYP2D6 inhibitor. Drugs that inhibit CYP2B6 (e.g. ticlopidine, clopidogrel) can significantly increase bupropion levels. Bupropion increases plasma concentrations of drugs metabolised by CYP2D6, including tricyclic antidepressants, antipsychotics, and certain beta blockers. Concurrent use with levodopa or amantadine may increase neurological side effects. MAOIs are absolutely contraindicated due to the risk of hypertensive crisis.
