INDICATION

Buphain 10mg Amp is an opioid analgesic indicated for the management of moderate to severe acute pain in clinical settings, including post-operative pain, trauma pain, labor pain, and cancer-related pain. It is also used as a pre-operative analgesic and as a supplement to balanced anaesthesia to provide intraoperative analgesia. As a mixed agonist-antagonist opioid, nalbuphine provides effective analgesia with a lower risk of euphoria and abuse potential compared to pure mu-opioid agonists such as morphine.

MECHANISM OF ACTION

Nalbuphine is a mixed opioid receptor agonist-antagonist that acts as a kappa-opioid receptor agonist and a partial antagonist at mu-opioid receptors. Kappa receptor agonism in the spinal cord and brain produces analgesia, sedation, and mild dysphoria, without the intense euphoria characteristic of mu-opioid agonists, which is the basis for its lower abuse potential. The partial mu-opioid antagonism means nalbuphine can precipitate withdrawal in patients physically dependent on pure mu-agonists such as morphine, pethidine, or fentanyl if given concurrently or shortly after. Respiratory depression from kappa agonism exhibits a ceiling effect at higher doses, providing a margin of safety compared to full mu-opioid agonists. Onset of action is rapid with intravenous injection (2 to 3 minutes) and duration of analgesia is 3 to 6 hours.

DOSAGE AND ADMINISTRATION

•      Adults: 10 to 20mg by intravenous, intramuscular, or subcutaneous injection every 3 to 6 hours as needed

•      Maximum single dose: 20mg; maximum daily dose: 160mg

•      Pre-operative and intraoperative use: 0.3 to 3mg/kg IV as directed by the anesthetist

•      Reduce dose in hepatic impairment and elderly patients; administer by trained healthcare professionals only

SIDE EFFECTS

Common:

•      Sedation and somnolence (most common; may be used therapeutically for post-operative sedation)

•      Nausea and vomiting

•      Dizziness and sweating

•      Dry mouth and headache

Less common:

•      Respiratory depression (ceiling effect limits severity, but monitor closely with CNS depressants)

•      Dysphoria and psychotomimetic effects such as hallucinations (kappa-opioid mediated)

•      Opioid withdrawal precipitation in patients dependent on pure mu-opioid agonists

•      Hypotension and bradycardia with rapid intravenous injection

NOT RECOMMENDED IN

•      Known hypersensitivity to nalbuphine or any excipient

•      Concurrent use with pure mu-opioid agonists in opioid-dependent patients (may precipitate acute withdrawal)

•      Severe respiratory depression or acute bronchial asthma without resuscitation facilities immediately available

•      Not for self-administration; use only under direct medical supervision

DRUG INTERACTIONS

CNS depressants including benzodiazepines, barbiturates, alcohol, and other opioids have additive sedative and respiratory depressant effects; reduce doses and monitor closely. Pure mu-opioid agonists such as morphine and fentanyl have their analgesic effects partially antagonized by nalbuphine due to its partial mu-antagonism; avoid combination or use with caution under specialist guidance. MAO inhibitors combined with opioids increase the risk of serotonin-like reactions including hyperthermia and CNS excitation; avoid concurrent use or within 14 days of stopping an MAO inhibitor. Nalbuphine does not interact significantly with CYP450 enzymes and has limited pharmacokinetic drug interactions.
