INDICATION

Brufen Duo 500/150mg is a fixed-dose combination NSAID and H2-receptor antagonist indicated for the relief of pain and inflammation in patients requiring regular NSAID therapy who are at risk of developing NSAID-associated gastrointestinal complications. It is used for osteoarthritis, rheumatoid arthritis, and other conditions where regular ibuprofen therapy is needed, with the famotidine component providing gastroprotection to reduce the incidence of NSAID-induced gastric and duodenal ulcers.

MECHANISM OF ACTION

Ibuprofen inhibits both COX-1 and COX-2 enzymes, reducing prostaglandin synthesis and providing anti-inflammatory, analgesic, and antipyretic effects. However, COX-1 inhibition reduces gastric mucosal prostaglandins that maintain mucosal integrity and promote bicarbonate and mucus secretion, increasing the risk of gastric erosions and ulcers. Famotidine is a competitive H2 histamine receptor antagonist that blocks histamine-stimulated gastric acid secretion by parietal cells. By reducing gastric acid production, famotidine maintains a higher intragastric pH that reduces the risk of NSAID-induced gastric and duodenal ulcer formation, allowing patients to benefit from ibuprofen analgesia with significantly lower gastrointestinal complication risk.

DOSAGE AND ADMINISTRATION

•      Adults: 1 tablet two to three times daily with food and water

SIDE EFFECTS

Common:

DRUG INTERACTIONS

Concurrent NSAIDs or low-dose aspirin increase gastrointestinal bleeding risk and reduce famotidine gastroprotective efficacy; avoid. Anticoagulants: warfarin effect enhanced; monitor INR. ACE inhibitors and ARBs may have reduced antihypertensive efficacy and renal function may worsen. Famotidine may reduce absorption of pH-dependent drugs including ketoconazole and atazanavir. Famotidine has less CYP450 interaction than cimetidine.
