INDICATION

Bronchilate Syrup 120ml is a beta-2 adrenergic bronchodilator in syrup form indicated for the relief of bronchospasm in bronchial asthma, chronic bronchitis, and other reversible obstructive airway diseases in adults and children. It is used for the symptomatic relief of acute wheeze, breathlessness, and chest tightness in patients who require oral bronchodilator therapy and cannot use or are not yet suitable for inhaled devices. The syrup formulation is particularly appropriate for young children and elderly patients.

MECHANISM OF ACTION

Salbutamol selectively stimulates beta-2 adrenergic receptors on bronchial smooth muscle cells, activating adenylyl cyclase and increasing intracellular cyclic AMP, which activates protein kinase A and inhibits myosin light chain kinase, resulting in smooth muscle relaxation and bronchodilation. Beta-2 stimulation also inhibits mast cell mediator release from bronchial mast cells and reduces mucosal oedema, providing additional bronchodilatory benefit. Salbutamol has high selectivity for beta-2 over beta-1 receptors, minimizing cardiac side effects compared to non-selective beta-agonists, and its systemic oral bioavailability makes the syrup an effective bronchodilator for patients unable to use inhalers.

DOSAGE AND ADMINISTRATION

•      Adults: 2 to 4mg (5 to 10ml) three to four times daily

•      Children 2 to 6 years: 1 to 2mg (2.5 to 5ml) three times daily

•      Children 6 to 12 years: 2mg (5ml) three times daily

•      Do not exceed the maximum recommended dose; always follow the dosage prescribed by your doctor or pediatrician

SIDE EFFECTS

Common:

•      Tremor of the hands (most common; dose-dependent)

•      Palpitations and tachycardia

•      Headache

•      Muscle cramps

Less common:

•      Hypokalaemia with high doses (particularly in combination with theophylline or diuretics; monitor serum potassium)

•      Peripheral vasodilation causing flushing and warmth

•      Hyperglycemia in diabetic patients with high doses

•      Paradoxical bronchospasm (rare; stop immediately if this occurs)

NOT RECOMMENDED IN

•      Known hypersensitivity to salbutamol or other beta-2 agonists

•      Threatened abortion or premature labor unless uterine relaxation is specifically required under obstetric supervision

•      Caution in patients with ischemic heart disease, arrhythmias, and hypertension

•      Not a substitute for inhaled therapy for asthma in patients who can use inhalers

DRUG INTERACTIONS

Beta-blockers (including eye drops) antagonize the bronchodilatory effect of salbutamol; avoid non-cardioselective beta-blockers in asthmatic patients. Corticosteroids combined with salbutamol at high doses increase hypokalaemia risk; monitor potassium. Digoxin combined with salbutamol increases arrhythmia risk from hypokalaemia. MAO inhibitors may potentiate cardiovascular effects. Theophylline and salbutamol have additive bronchodilatory and additive hypokalaemia risk.
