INDICATION

Brivera 50mg Tab is a third-generation anticonvulsant used as adjunctive therapy for partial-onset (focal) seizures with or without secondary generalization in adults, adolescents, and children aged 4 years and above. It may be used as monotherapy in adults when other antiepileptic drugs have not provided adequate seizure control.

MECHANISM OF ACTION

Brivaracetam is a high-affinity ligand for synaptic vesicle protein 2A (SV2A), a glycoprotein involved in regulating vesicle exocytosis at the synapse. By binding to SV2A, brivaracetam modulates neurotransmitter release and reduces neuronal hyperexcitability, thereby preventing the abnormal synchronized electrical discharges that cause seizures. It has 10-fold higher affinity for SV2A than levetiracetam, and also partially inhibits voltage-gated sodium channels.

DOSAGE AND ADMINISTRATION

•      Adults and adolescents: starting dose 50mg twice daily; may be adjusted to 25mg or 100mg twice daily

•      Children aged 4 to 16 years (20kg and above): dose based on weight; typically, 25mg to 100mg twice daily

•      Take with or without food; take regularly at the same times each day

•      If switching from levetiracetam, no washout period is needed

•      Dose adjustments required in patients with hepatic impairment

SIDE EFFECTS

Common:

•      Somnolence (drowsiness), dizziness

•      Fatigue, nausea

•      Headache

•      Irritability or mood changes

Less common:

•      Psychiatric symptoms including aggression, agitation, anxiety, and depression

•      Suicidal ideation (all antiepileptics carry this risk — monitor closely)

•      Coordination problems (ataxia, dysarthria)

•      Stevens-Johnson syndrome (rare skin reaction)

NOT RECOMMENDED IN

•      Known hypersensitivity to brivaracetam or any excipient

•      Use with caution in patients with a history of psychiatric disorders

•      Not recommended during pregnancy unless the benefit clearly outweighs the risk

•      Abrupt withdrawal should be avoided; taper dose gradually

DRUG INTERACTIONS

Rifampicin, a strong CYP2C19 inducer, reduces brivaracetam exposure by approximately 45%; dose adjustment may be required. Brivaracetam inhibits CYP2C19 and may increase plasma levels of phenytoin and other CYP2C19 substrates. Co-administration with carbamazepine may increase carbamazepine-epoxide levels. No clinically relevant interactions with levetiracetam, lamotrigine, or valproate have been reported.
