INDICATION
Bozpah 62.5mg Tab is an endothelin receptor antagonist (ERA) indicated for the treatment of pulmonary arterial hypertension (PAH, WHO Group I) in adults to improve exercise capacity and delay clinical worsening. It is used in patients with idiopathic or heritable PAH, PAH associated with connective tissue disease (scleroderma, lupus), and PAH associated with congenital heart disease. The 62.5mg dose is the initial starting dose used for the first 4 weeks of therapy before up-titration to the maintenance dose.
MECHANISM OF ACTION
Bosentan is a competitive antagonist at both endothelin-A (ETA) and endothelin-B (ETB) receptors. Endothelin-1 is a potent vasoconstrictor and mitogen that plays a central role in PAH pathogenesis by causing pulmonary vasoconstriction, vascular smooth muscle proliferation, and vascular remodeling. By blocking both ETA and ETB receptors in the pulmonary vasculature, bosentan reduces pulmonary vascular resistance, lowers mean pulmonary arterial pressure, and reduces right ventricular afterload, improving exercise tolerance, hemodynamics, and time to clinical worsening in PAH.
DOSAGE AND ADMINISTRATION
• Starting dose: 62.5mg twice daily for 4 weeks
• Maintenance dose (after 4 weeks): increase to 125mg twice daily if tolerated
• Take with or without food; take consistently twice daily, 12 hours apart
• Mandatory liver function monitoring (monthly LFTs) throughout therapy; pregnancy must be excluded before starting and reliable contraception maintained during and for 1 month after stopping
SIDE EFFECTS
Common:
• Elevated liver aminotransferases (AST/ALT) in approximately 10% of patients; dose-dependent and reversible on discontinuation
• Headache and flushing
• Nasopharyngitis
• Peripheral oedema and fluid retention
Less common:
• Teratogenicity: bosentan causes major birth defects; absolutely contraindicated in pregnancy
• Hematological effects: anemia and decreased hematocrit (monitor hemoglobin)
• Palpitations and hypotension
• Hepatic toxicity with elevated bilirubin requiring treatment discontinuation
NOT RECOMMENDED IN
• Pregnancy (major teratogen; Category X; both male and female patients must use reliable contraception)
• Severe hepatic impairment
• Concurrent use with cyclosporine A or glyburide
• Known hypersensitivity to bosentan or any excipient
DRUG INTERACTIONS
Cyclosporine A is contraindicated; co-administration increases bosentan levels by 30-fold due to OATP inhibition. Glyburide combined with bosentan increases hepatotoxicity risk; contraindicated. Bosentan is a potent inducer of CYP3A4 and CYP2C9 and significantly reduces plasma levels of drugs metabolized by these enzymes including warfarin, statins, sildenafil, and hormonal contraceptives; monitor INR and adjust doses. Rifampicin reduces bosentan levels; avoid combination.
