INDICATION

Boferin 500mg/10ml Inj is a high-dose intravenous iron preparation indicated for the treatment of iron-deficiency anemia in patients where oral iron therapy is insufficient, not tolerated, or not clinically feasible. It is specifically indicated in patients with chronic kidney disease, heart failure, inflammatory bowel disease, post-partum iron deficiency anemia, and in patients with severe iron deficiency requiring rapid iron repletion. Its ability to deliver up to 1000mg of iron in a single dose makes it particularly efficient.

MECHANISM OF ACTION

Ferric carboxymaltose is an iron-carbohydrate complex where polynuclear iron (III)-hydroxide is stabilized by a carboxymaltose shell. After intravenous administration, the complex is taken up by the reticuloendothelial system (primarily liver, spleen, and bone marrow macrophages) via endocytosis. The carboxymaltose shell is gradually metabolized, releasing iron which binds to transferrin and is transported to erythroid precursors for hemoglobin synthesis. The slow, controlled release of iron from the carboxymaltose complex allows large doses to be given safely in a short infusion, minimizing free iron toxicity and anaphylaxis risk.

DOSAGE AND ADMINISTRATION

      Adults: up to 1000mg iron (2 vials of 500mg) by slow IV infusion over 15 minutes, once weekly until the calculated total iron deficit is replenished

      Maximum single dose: 1000mg iron

      Administer by slow intravenous injection or infusion; never administer intramuscularly

      All doses calculated using the Ganzoni formula based on body weight and baseline hemoglobin; prescribed and administered by healthcare professionals

SIDE EFFECTS

Common:

      Hypophosphatasemia (low phosphate) with repeated dosing; monitor serum phosphate

      Nausea, vomiting, and abdominal pain

      Headache and dizziness

      Transient hypotension with rapid infusion

Less common:

      Hypersensitivity reactions including anaphylactoid reactions (rare but serious; have resuscitation equipment available)

      Skin discoloration and pruritus at the injection site if extravasation occurs

      Arthralgia and myalgia

      Iron overload with excessive cumulative dosing

NOT RECOMMENDED IN

      Iron overload conditions (haemochromatosis)

      Anemia not caused by iron deficiency

      Known hypersensitivity to ferric carboxymaltose or any excipient

      First trimester of pregnancy

DRUG INTERACTIONS

Oral iron supplements should not be started within 5 days of intravenous iron to avoid interaction with transferrin binding sites. ACE inhibitors may increase systemic reactions to intravenous iron preparations. Do not mix with other medications or solutions other than normal saline.
