INDICATION

Bofalgan Plus Inj is a combination injectable analgesic indicated for the treatment of moderate to moderately severe acute pain in clinical settings where both central and peripheral pain pathways need to be addressed. It is used for post-operative pain, trauma pain, musculoskeletal pain requiring parenteral therapy, and acute pain episodes in hospitalized patients where single-agent therapy is insufficient.

MECHANISM OF ACTION

Paracetamol inhibits central cyclooxygenase enzymes, reducing prostaglandin synthesis in the CNS and providing analgesic and antipyretic effects through central pain modulation pathways. Tramadol is a weak mu-opioid receptor agonist that also inhibits the neuronal reuptake of serotonin and norepinephrine, enhancing descending inhibitory pain pathways. The combination provides synergistic analgesia through complementary central mechanisms, allowing effective pain control with lower doses of each individual component compared to monotherapy, reducing opioid-related side effects while maintaining adequate analgesia.

DOSAGE AND ADMINISTRATION

•      Adults: 1 to 2ml (containing paracetamol 300 to 600mg and tramadol 25 to 50mg) by slow intravenous or intramuscular injection every 4 to 6 hours

•      Do not exceed 6 ampoules (tramadol 150mg) per day from this preparation alone

•      Monitor total paracetamol intake from all sources; maximum 4g/day from all paracetamol preparations

•      Administer by trained healthcare professionals; not for self-administration

SIDE EFFECTS

Common:

•      Nausea and vomiting (tramadol and paracetamol component)

•      Dizziness and somnolence (tramadol)

•      Dry mouth

•      Constipation

Less common:

•      Serotonin syndrome when combined with serotonergic drugs (tramadol component)

•      Seizures at high tramadol doses

•      Dependency and withdrawal with prolonged tramadol use

•      Hepatotoxicity with paracetamol overdose

NOT RECOMMENDED IN

•      Acute intoxication with alcohol, opioids, or CNS depressants

•      Concurrent MAO inhibitors (hypertensive crisis and serotonin syndrome risk)

•      Severe renal or hepatic impairment

•      Known hypersensitivity to paracetamol or tramadol; epilepsy not adequately controlled

DRUG INTERACTIONS

MAO inhibitors are contraindicated within 14 days of tramadol. SSRIs, SNRIs, triptans, and linezolid increase serotonin syndrome risk. CNS depressants and opioids have additive respiratory depression and sedation. Warfarin: monitor INR with paracetamol. CYP2D6 inhibitors reduce tramadol conversion to active M1 metabolite.
