INDICATION

Blephis 5ml Eye Drop is a topical ophthalmic corticosteroid indicated for the treatment of steroid-responsive inflammatory conditions of the palpebral and bulbar conjunctiva, cornea, and anterior segment of the globe. It is used in allergic conjunctivitis, vernal conjunctivitis, anterior uveitis, episcleritis, keratitis, and post-operative inflammation following cataract extraction and other intraocular surgical procedures. Prednisolone acetate 1% is one of the most potent topical ophthalmic corticosteroids and is preferred for moderate to severe ocular inflammation requiring rapid and robust anti-inflammatory control.

MECHANISM OF ACTION

Prednisolone acetate is a synthetic corticosteroid prodrug that penetrates the corneal epithelium and is hydrolysed to the active prednisolone within ocular tissue. It binds to intracellular glucocorticoid receptors in conjunctival, corneal, and uveal cells, modulating transcription of anti-inflammatory genes and suppressing the production of prostaglandins, leukotrienes, histamine, and pro-inflammatory cytokines. It inhibits phospholipase A2, preventing arachidonic acid release and subsequent inflammatory mediator cascade. At 1% concentration, prednisolone acetate is the most potent topical ophthalmic steroid available, providing more complete suppression of ocular inflammation than lower-potency agents such as fluorometholone, making it appropriate for conditions requiring maximal anti-inflammatory efficacy.

DOSAGE AND ADMINISTRATION

      Acute inflammation: Instil 1 to 2 drops into the affected eye(s) every 1 to 2 hours during waking hours for the first 24 to 48 hours

      Moderate inflammation: 1 to 2 drops 4 times daily; taper frequency based on clinical response

      Shake the bottle well before each use as this is a suspension

      Do not use for more than the prescribed duration without ophthalmological review; always follow your ophthalmologist's instructions

SIDE EFFECTS

Common:

      Transient stinging or burning on instillation

      Temporary blurring of vision after instillation

      Eye irritation and foreign body sensation

      Mild conjunctival hyperemia

Less common:

      Elevated intraocular pressure (steroid-induced glaucoma) with prolonged use; monitor IOP regularly

      Posterior subcapsular cataracts with prolonged corticosteroid use

      Secondary bacterial, viral, or fungal ocular infection due to immunosuppression

      Delayed corneal wound healing after ocular surgery

NOT RECOMMENDED IN

      Epithelial herpes simplex keratitis (dendritic keratitis) and other active viral diseases of the cornea and conjunctiva

      Mycobacterial ocular infections and fungal diseases of the eye

      Known hypersensitivity to prednisolone or any corticosteroid

      Remove contact lenses before instillation; wait at least 15 minutes before reinserting

DRUG INTERACTIONS

Topical ophthalmic corticosteroids have minimal systemic absorption and no clinically significant systemic drug interactions at standard doses. If using concurrent topical ophthalmic antibiotics or other eye drops, Instil them at least 5 minutes apart to prevent dilution and washout. Concurrent use of topical NSAIDs with prednisolone eye drops may reduce anti-inflammatory efficacy and increase risk of corneal complications; use with caution and under ophthalmologist supervision.

STORAGE INSTRUCTIONS

Store between 2°C and 25°C. Shake well before use. Do not freeze. Protect from direct sunlight. Discard 28 days after opening. Keep out of reach of children.
