INDICATION
Azitma 500mg Tab is a macrolide antibiotic prescribed for moderate to severe community-acquired pneumonia, acute bacterial exacerbations of chronic obstructive pulmonary disease, pelvic inflammatory disease, typhoid fever, and severe skin infections. The 500mg strength is also commonly used as the loading dose in five-day antibiotic regimens.
MECHANISM OF ACTION
Azithromycin binds to the 50S ribosomal subunit of susceptible bacteria and inhibits RNA-dependent protein synthesis. This bacteriostatic action prevents bacterial growth. Azithromycin has a uniquely long tissue half-life due to its ability to concentrate in phagocytes and macrophages, which transport it to infection sites. It is active against a broad range of Gram-positive and Gram-negative organisms, atypical pathogens such as Chlamydia, Mycoplasma, and Legionella, and certain anaerobes.
DOSAGE AND ADMINISTRATION
• Community-acquired pneumonia (step-down therapy): 500mg once daily for 3 days
• Standard 5-day regimen: 500mg on Day 1, then 250mg once daily on Days 2 to 5
• Pelvic inflammatory disease: 500mg IV/oral once daily for 1 to 2 days, then 250mg orally for 5 to 7 days
• Take at least 1 hour before or 2 hours after meals
• Swallow tablet whole with water
SIDE EFFECTS
Common:
• Nausea, vomiting, diarrhea
• Abdominal cramps
• Headache, dizziness
• Altered taste
Less common:
• QT interval prolongation and ventricular arrhythmias
• Hepatotoxicity and abnormal liver function tests
• Severe skin reactions (Stevens-Johnson syndrome, toxic epidermal necrolysis)
• Hearing loss or tinnitus with prolonged high doses
NOT RECOMMENDED IN
• Known hypersensitivity to azithromycin or other macrolide antibiotics
• History of hepatic dysfunction from previous azithromycin use
• Known QT prolongation, hypokalaemia, or hypomagnesaemia
• Severe hepatic impairment
DRUG INTERACTIONS
Co-administration with QT-prolonging drugs such as antipsychotics, halofantrine, and amiodarone increases the risk of cardiac arrhythmias. Aluminium- and magnesium-containing antacids reduce peak azithromycin plasma concentrations; administer 2 hours apart. Azithromycin may increase INR in patients on warfarin. Concomitant use with colchicine or ergot derivatives may increase toxicity. Azithromycin modestly inhibits P-glycoprotein and may increase exposure to P-gp substrates.
