INDICATION

Atropine Inj is an anticholinergic medication used to treat symptomatic bradycardia (slow heart rate), as a pre-anaesthetic agent to reduce excessive salivation and respiratory secretions during surgery, in the management of organophosphate poisoning and nerve agent exposure, and to reverse the effects of cholinergic drugs such as neostigmine and pyridostigmine. It is also used during cardiopulmonary resuscitation.

MECHANISM OF ACTION

Atropine is a competitive antagonist at muscarinic acetylcholine receptors. By blocking these receptors, it inhibits the parasympathetic nervous system, producing increases in heart rate, reduction in glandular secretions, relaxation of smooth muscle in the gastrointestinal and urinary tracts, and dilation of the pupils. In the setting of bradycardia, it blocks vagal tone on the sinoatrial node, restoring a normal heart rate.

DOSAGE AND ADMINISTRATION

•      Bradycardia: 0.5mg to 1mg IV every 3 to 5 minutes; maximum total dose 3mg

•      Pre-anaesthesia: 0.3mg to 0.6mg IM or IV 30 to 60 minutes before induction

•      Organophosphate poisoning: 2mg to 4mg IV every 5 to 10 minutes until secretions dry

•      Reversal of neuromuscular blockade: 0.6mg to 1.2mg IV with neostigmine

•      Pediatric bradycardia: 0.02mg/kg IV; minimum dose 0.1mg to avoid paradoxical bradycardia

SIDE EFFECTS

Common:

•      Dry mouth, blurred vision, urinary retention

•      Increased heart rate (tachycardia)

•      Flushing, dry skin

•      Constipation, reduced bowel sounds

Less common:

•      Confusion and agitation, especially in elderly patients

•      Raised intraocular pressure, precipitating acute glaucoma

•      Ventricular fibrillation at very high doses

•      Hyperthermia due to decreased sweating

NOT RECOMMENDED IN

•      Narrow-angle glaucoma

•      Obstructive uropathy or bladder outflow obstruction

•      Tachyarrhythmias and atrial flutter or fibrillation with rapid ventricular response

•      Known hypersensitivity to atropine or belladonna alkaloids

•      Use with caution in patients with myasthenia gravis

DRUG INTERACTIONS

Concurrent use with other anticholinergic drugs such as antihistamines, tricyclic antidepressants, and phenothiazines amplifies the anticholinergic side effects, increasing the risk of urinary retention, constipation, and confusion. Antacids and adsorbents can reduce the oral absorption of atropine. Metoclopramide's prokinetic effects on the gastrointestinal tract are antagonized by atropine. Atropine can increase the absorption of certain other drugs by reducing gastric motility. Monoamine oxidase inhibitors may intensify anticholinergic effects.
