INDICATION

Artelac Nighttime Gel 10gm is a sterile ophthalmic viscous gel indicated for the management of dry eye disease, particularly for nocturnal use when prolonged lubricating action is required. It is indicated for moderate to severe dry eye disease, nocturnal lagophthalmos (incomplete eyelid closure during sleep), and patients who experience significant worsening of dry eye symptoms overnight or upon waking. The thick gel formulation provides sustained lubrication throughout the night without the need for repeat application.

MECHANISM OF ACTION

Carbomer (polyacrylic acid) is a high molecular weight polymer that forms a viscous, mucoadhesive gel with water at physiological pH. This gel closely mimics the mucin layer of the tear film in its bio adhesive properties, adsorbing to the ocular surface and providing sustained lubrication and hydration throughout the night. The viscous nature of the gel reduces the rate of drainage from the ocular surface; prolonging contact time compared to eye drops and providing protection against overnight desiccation. The gel stabilises and supplements the residual tear film, protecting the corneal and conjunctival epithelium from the friction and drying that can occur with incomplete eyelid closure.

DOSAGE AND ADMINISTRATION

•      Apply a small amount (about a 1cm strip) to the inner aspect of the lower eyelid at bedtime

•      Gently close the eye and allow the gel to spread over the ocular surface

•      The gel will cause temporary blurring of vision; apply only at bedtime and avoid driving after application

•      Wash hands thoroughly before application; do not touch the tube tip to any surface

SIDE EFFECTS

Common:

•      Significant temporary blurring of vision (expected from the gel formulation; apply only at bedtime)

•      Mild stinging or burning on application in some patients

•      Sensation of foreign body in the eye briefly after application

•      Sticky eyelids on waking (residual gel)

Less common:

•      Allergic conjunctivitis in individuals sensitive to any excipient

•      Eye irritation persisting beyond initial blurring

•      Hypersensitivity to carbomer or benzalkonium chloride (if present as preservative)

•      Superficial punctate keratopathy with chronic overuse

NOT RECOMMENDED IN

•      Not for use as a daytime drop due to significant sustained vision blurring

•      Known hypersensitivity to carbomer or any excipient

•      Remove contact lenses before applying; do not use with contact lenses

•      If eye pain, change in vision, or continued redness occurs after use, consult an ophthalmologist

DRUG INTERACTIONS

Ophthalmic gels have no systemic drug interactions. If other topical ophthalmic medications are used during the day, the nighttime gel should be applied last, at least 15 minutes after other drops. The viscous gel will reduce the bioavailability of another eye drops if applied simultaneously.
