INDICATION

Ardcil 4.5g Inj is a broad-spectrum combination antibiotic used for the treatment of serious bacterial infections caused by piperacillin-resistant, beta-lactamase-producing organisms. It is indicated for intra-abdominal infections (appendicitis, peritonitis), hospital-acquired pneumonia, skin and soft tissue infections, urinary tract infections (including pyelonephritis), gynaecological infections (including endometritis and pelvic inflammatory disease), bacteraemia associated with the above infections, and febrile neutropenia in immunocompromised patients.

MECHANISM OF ACTION

Ardcil 4.5g Inj combines piperacillin, an extended-spectrum penicillin antibiotic, with tazobactam, a beta-lactamase inhibitor. Piperacillin exerts bactericidal activity by binding to penicillin-binding proteins (PBPs) on bacterial cell walls, inhibiting the cross-linking of peptidoglycan chains and disrupting cell wall synthesis. This leads to bacterial cell lysis and death. Tazobactam alone has minimal antibacterial activity, but it irreversibly inactivates many beta-lactamase enzymes produced by resistant organisms, thereby protecting piperacillin from enzymatic degradation and restoring its bactericidal activity against resistant strains.

DOSAGE AND ADMINISTRATION

•      Adults: 4.5g intravenous infusion every 6 to 8 hours (3 to 4 times daily)

•      Severe infections including nosocomial pneumonia: 4.5g every 6 hours

•      Febrile neutropenia: 4.5g every 6 hours in combination with an aminoglycoside

•      Renal impairment: dose adjustment required based on creatinine clearance

•      Each 4.5g vial should be reconstituted with 20ml of compatible diluent and administered via IV infusion over 30 minutes

SIDE EFFECTS

Common:

•      Diarrhoea, nausea, vomiting

•      Rash and skin reactions

•      Phlebitis or pain at the infusion site

•      Headache

Less common:

•      Hypokalaemia (low potassium levels) with prolonged high-dose therapy

•      Elevated liver enzymes and hepatotoxicity

•      Clostridium difficile-associated diarrhoea (pseudomembranous colitis)

•      Neuromuscular excitability or seizures at very high doses

•      Leucopenia or thrombocytopenia with extended courses

NOT RECOMMENDED IN

•      Known hypersensitivity to piperacillin, tazobactam, penicillins, or cephalosporins

•      History of severe anaphylactic reaction to any beta-lactam antibiotic

•      Use with extreme caution in patients with severe renal impairment

•      Physically incompatible with aminoglycosides — do not mix in the same infusion bag or syringe

DRUG INTERACTIONS

Piperacillin/tazobactam can reduce the renal tubular secretion of methotrexate, leading to prolonged and elevated methotrexate plasma concentrations with risk of toxicity. Probenecid inhibits the renal excretion of piperacillin and tazobactam, increasing and prolonging their plasma concentrations. Combined use with nephrotoxic agents such as vancomycin may increase the risk of acute kidney injury, particularly in critically ill patients. Concurrent use with anticoagulants such as heparin or warfarin may require more frequent monitoring as piperacillin may affect platelet aggregation at high doses. Piperacillin/tazobactam is physically incompatible with aminoglycosides such as gentamicin and tobramycin; administer separately.

STORAGE INSTRUCTIONS

Store Ardcil 4.5g Inj vials at room temperature below 25°C. Protect from light. After reconstitution, the solution is stable for 12 hours at room temperature or 48 hours when refrigerated at 2°C to 8°C. Discard any unused portion. Keep out of reach of children.
