INDICATION
Apreon 80/125mg Cap is an antiemetic medication used to prevent chemotherapy-induced nausea and vomiting (CINV), including both the acute phase (within 24 hours) and the delayed phase (after 24 hours) following highly emetogenic chemotherapy (HEC) regimens such as cisplatin. It is also indicated as part of a combination regimen for moderately emetogenic chemotherapy (MEC). Additionally, it may be used for the prevention of postoperative nausea and vomiting (PONV).
MECHANISM OF ACTION
Aprepitant is a highly selective neurokinin-1 (NK1) receptor antagonist. It blocks the action of substance P — a neuropeptide that binds to NK1 receptors in the brain's vomiting centre (chemoreceptor trigger zone and nucleus tractus solitarius). Cytotoxic chemotherapy stimulates substance P release, which activates NK1 receptors and triggers the emetic reflex. By blocking these receptors, aprepitant suppresses both the acute and delayed phases of chemotherapy-induced emesis. It is used alongside serotonin (5-HT3) antagonists and corticosteroids for optimal antiemetic control.
DOSAGE AND ADMINISTRATION
• Day 1 (before chemotherapy): 125mg capsule taken orally 1 hour before chemotherapy
• Day 2 and Day 3: 80mg capsule taken once daily in the morning
• Apreon is always used as part of a combination antiemetic regimen
• Capsules may be taken with or without food
• No dose adjustment needed in elderly patients or those with mild to moderate renal impairment
SIDE EFFECTS
Common:
• Fatigue and general weakness
• Hiccups
• Constipation, diarrhoea, or nausea
• Loss of appetite (anorexia)
Less common:
• Elevated liver enzymes (ALT, AST)
• Headache and dizziness
• Dehydration from inadequate antiemetic control
• Alopecia (rare, likely related to chemotherapy)
NOT RECOMMENDED IN
• Known hypersensitivity to aprepitant or any component of the formulation
• Concurrent use with pimozide, terfenadine, astemizole, or cisapride (risk of serious arrhythmia)

DRUG INTERACTIONS
Aprepitant is both a substrate and a moderate inhibitor of CYP3A4. It can increase plasma levels of drugs metabolised by CYP3A4, including chemotherapy agents such as docetaxel, paclitaxel, etoposide, ifosfamide, imatinib, and vinorelbine, potentially increasing their toxicity. Dexamethasone doses should be reduced by approximately 50% when co-administered with aprepitant due to increased systemic exposure. Aprepitant reduces the efficacy of warfarin and oral contraceptives by inducing CYP2C9 and CYP3A4. Rifampicin and other strong CYP3A4 inducers significantly reduce aprepitant plasma concentrations.
