INDICATION

Apixaget 2.5mg Tab is an oral anticoagulant (blood thinner) prescribed to prevent and treat blood clots. It is indicated for the prevention of venous thromboembolic events (VTE) following elective hip or knee replacement surgery, prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation, treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE), and prevention of recurrent DVT and PE.

MECHANISM OF ACTION

Apixaban is a potent, oral, reversible, and highly selective direct inhibitor of Factor Xa — a critical enzyme in the coagulation cascade. By binding directly to the active site of Factor Xa, apixaban inhibits both free and clot-bound Factor Xa as well as prothrombinase activity. This prevents the conversion of prothrombin to thrombin, which in turn blocks fibrin clot formation. Unlike warfarin, apixaban does not require antithrombin III for its anticoagulant activity and does not directly affect platelet aggregation.

DOSAGE AND ADMINISTRATION

•      Post-surgical VTE prevention (knee replacement): 2.5mg twice daily for 10 to 14 days

•      Post-surgical VTE prevention (hip replacement): 2.5mg twice daily for 32 to 38 days

•      Atrial fibrillation (stroke prevention): 2.5mg twice daily (if at least 2 of: age ≥80, weight ≤60kg, serum creatinine ≥1.5mg/dL)

•      Tablets may be taken with or without food

•      Swallow whole with water; do not crush or chew

SIDE EFFECTS

Common:

•      Bruising, nosebleeds, gum bleeding

•      Haematuria (blood in urine)

•      Gastrointestinal bleeding

•      Anaemia due to occult blood loss

Less common:

•      Hypersensitivity reactions including skin rash

•      Elevated liver enzymes

•      Spinal or epidural haematoma with neuraxial anaesthesia

•      Excessive bleeding from cuts, wounds, or surgical sites

NOT RECOMMENDED IN

•      Active pathological bleeding or significant risk of major bleeding

•      Hepatic disease associated with coagulopathy

•      Prosthetic heart valves requiring anticoagulation

•      Pregnancy and breastfeeding

•      Severe renal impairment (creatinine clearance <15ml/min)

DRUG INTERACTIONS

Apixaban plasma levels are significantly increased by strong dual inhibitors of CYP3A4 and P-glycoprotein, such as ketoconazole, itraconazole, ritonavir, and clarithromycin, raising the risk of bleeding. Conversely, strong inducers of these pathways, including rifampicin, phenytoin, carbamazepine, and St John's Wort, reduce apixaban concentrations and diminish its anticoagulant effect. Combined use with other anticoagulants, antiplatelets, fibrinolytics, NSAIDs, or aspirin increases the risk of haemorrhage. Antidote: Andexanet alfa is a reversal agent for life-threatening bleeding on apixaban.
