INDICATION
Amrovil Injection is a first-generation antihistamine used in the management of allergic conditions including hay fever, allergic rhinitis, urticaria (hives), angioedema, conjunctivitis, and skin reactions such as redness and itching. It is also used as an adjunct in the treatment of anaphylaxis and severe allergic reactions when given alongside epinephrine. The injectable form is particularly useful in acute allergic emergencies requiring rapid onset of action.
MECHANISM OF ACTION
Pheniramine maleate acts as a competitive antagonist at peripheral H1 histamine receptors. It competes with histamine for binding at effector cell receptor sites on blood vessels, bronchial smooth muscle, and secretory glands. By occupying these receptors, it prevents histamine from triggering vasodilation, increased vascular permeability, bronchoconstriction, and mucus secretion. Pheniramine also has anticholinergic properties and mild sedative effects due to its partial central nervous system penetration.
DOSAGE AND ADMINISTRATION
• Adults: 0.5ml to 1ml (approximately 22.75mg) given intravenously (IV) at 1ml/min, or intramuscularly (IM), once or twice daily
• Children (6 months to 3 years): 0.4ml to 1ml IM once or twice daily
• Children (over 4 years): 0.8ml to 2ml IM once or twice daily
• Intravenous doses should be administered slowly under medical supervision
• Dose and frequency adjusted based on severity of the allergic reaction
SIDE EFFECTS
Common:
• Drowsiness and sedation
• Dry mouth, nose, and throat
• Urinary retention
• Gastrointestinal upset
Less common:
• Palpitations and tachycardia at higher doses
• Hallucinations, agitation, or disorientation, especially in children
• Increased intraocular pressure
• Blurred vision
NOT RECOMMENDED IN
• Prostatic hypertrophy (enlarged prostate gland)
• Known hypersensitivity to pheniramine or related antihistamines
• Narrow-angle glaucoma
• Should not be used in pregnancy without medical advice
• Caution in patients with severe liver disease
DRUG INTERACTIONS
Pheniramine maleate has additive CNS depression effects when combined with alcohol, sedatives, hypnotics, opioids, or other CNS depressants, which can significantly increase the risk of excessive sedation. Concurrent use with monoamine oxidase inhibitors (MAOIs) can prolong and intensify the anticholinergic effects of pheniramine. Combination with tricyclic antidepressants amplifies anticholinergic side effects such as dry mouth, urinary retention, and constipation. Patients taking anticholinergic medications such as atropine should be monitored carefully for additive adverse effects.
STORAGE INSTRUCTIONS
Store Amrovil Injection at room temperature between 15°C and 25°C. Protect from light. Do not freeze. Discard unused portions of the vial after use. Keep out of reach of children.
