INDICATION

Amrolam 0.5mg Tab is a short-term prescription medication used to manage anxiety disorders and panic disorder, including panic attacks with or without agoraphobia. It helps reduce excessive worry, tension, restlessness, and the physical symptoms of anxiety such as muscle tightness and difficulty sleeping. It may also be prescribed for anxiety associated with depression. Due to its potential for dependence, it is intended for short-term use only under medical supervision.

MECHANISM OF ACTION

Alprazolam belongs to the benzodiazepine class of medications. It enhances the effect of gamma-aminobutyric acid (GABA), the brain's main inhibitory neurotransmitter, by binding to specific sites on GABA-A receptors. This increases the frequency of chloride ion channel opening, which hyperpolarises neurons and reduces their excitability. The result is a calming effect on the central nervous system, reducing anxiety, muscle tension, and preventing panic episodes.

DOSAGE AND ADMINISTRATION

•      Anxiety disorders: 0.25mg to 0.5mg three times daily; may be increased gradually

•      Panic disorder: 0.5mg three times daily; may be increased up to 10mg/day in divided doses

•      Elderly or debilitated patients: start at 0.25mg two to three times daily

•      Tablets should be taken at regular intervals as directed by a doctor

•      Do not stop abruptly; taper the dose gradually under medical guidance

SIDE EFFECTS

Common:

•      Drowsiness and sedation

•      Dizziness, light-headedness

•      Impaired coordination and memory

•      Slurred speech

Less common:

•      Dependence and withdrawal symptoms with prolonged use

•      Depression or worsening mood in some patients

•      Paradoxical reactions including agitation or aggression

•      Respiratory depression, especially at higher doses or with alcohol

NOT RECOMMENDED IN

•      Acute narrow-angle glaucoma

•      Known hypersensitivity to alprazolam or other benzodiazepines

•      Concurrent use with strong CYP3A4 inhibitors (e.g., itraconazole, ketoconazole)

•      Pregnancy, particularly the first trimester — risk of foetal harm

•      Should not be used in patients with severe respiratory insufficiency

DRUG INTERACTIONS

Alprazolam is metabolised primarily by the CYP3A4 enzyme. Strong inhibitors of this enzyme, such as itraconazole, ketoconazole, fluoxetine, and nefazodone, can significantly increase plasma alprazolam concentrations, intensifying its sedative effects. Rifampicin and carbamazepine are inducers that reduce alprazolam levels, diminishing its efficacy. Combined use with alcohol, opioids, antihistamines, or other CNS depressants greatly increases the risk of respiratory depression, sedation, and overdose. Cimetidine can also elevate alprazolam levels by reducing its hepatic clearance.
