INDICATION

Amgydex Oint 3.5g is a combined ophthalmic antibiotic and corticosteroid ointment indicated for steroid-responsive inflammatory conditions of the eye where a bacterial infection or risk of bacterial infection exists. It is used in bacterial ocular infections with significant inflammation including bacterial conjunctivitis with inflammatory reaction, blepharitis, corneal ulcers with bacterial superinfection, and post-operative conditions following cataract surgery and other ocular procedures where anti-infective and anti-inflammatory therapy are both required.

MECHANISM OF ACTION

Tobramycin is an aminoglycoside antibiotic that inhibits bacterial protein synthesis by binding to the 30S ribosomal subunit, interfering with mRNA translation and causing misreading of the genetic code, resulting in bactericidal activity against a broad spectrum of gram-positive and gram-negative ocular pathogens including Staphylococcus aureus, Pseudomonas aeruginosa, Haemophilus influenzae, and Streptococcus pneumoniae. Dexamethasone is a potent corticosteroid that suppresses the inflammatory response in ocular tissue by reducing prostaglandin and cytokine production, controlling oedema, hyperemia, and cellular infiltration. The ointment base provides prolonged ocular contact time and sustained release of both active ingredients.

DOSAGE AND ADMINISTRATION

•      Apply a small amount (approximately half an inch ribbon) into the conjunctival sac of the affected eye every 6 to 8 hours

•      During the first 24 to 48 hours, dosing frequency may be increased

•      Gently pull down the lower eyelid and apply the ointment without touching the tube tip to the eye or any surface

•      Do not use for longer than the prescribed duration; always follow your ophthalmologist's instructions

SIDE EFFECTS

Common:

•      Temporary blurring of vision immediately after application (the ointment base causes this; clear within minutes)

•      Mild burning or stinging on application

•      Eye irritation and foreign body sensation

•      Mild conjunctival hyperemia

Less common:

•      Elevated intraocular pressure with prolonged steroid use; monitor IOP

•      Posterior subcapsular cataracts with prolonged corticosteroid use

•      Secondary fungal or viral ocular infection (corticosteroid masking of signs)

•      Aminoglycoside-related corneal epithelial toxicity with excessive or prolonged use

NOT RECOMMENDED IN

•      Epithelial herpes simplex keratitis (dendritic keratitis) and active viral ocular infections

•      Mycobacterial ocular infections and fungal diseases of the eye

•      Known hypersensitivity to tobramycin or dexamethasone

•      Do not use contact lenses during treatment; remove before application and wait 15 minutes before reinserting

DRUG INTERACTIONS

Topical ophthalmic preparations have minimal systemic absorption and no significant systemic drug interactions. If using multiple topical ophthalmic preparations, apply at least 5 minutes apart to avoid dilution and washout of active ingredients.

STORAGE INSTRUCTIONS

Store between 2°C and 25°C. Protect from light. Do not freeze. Keep tube tightly closed. Discard 4 weeks after first opening. Keep out of reach of children.
