INDICATION

Aine 50mg Tab is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of major depressive disorder, obsessive-compulsive disorder (OCD), panic disorder with or without agoraphobia, post-traumatic stress disorder (PTSD), social anxiety disorder, and premenstrual dysphoric disorder (PMDD). It is one of the most widely prescribed and evidence-supported SSRIs available, with established efficacy across a broad range of anxiety and depressive disorders in adults and children (for OCD, in children 6 years and above).

MECHANISM OF ACTION

Sertraline selectively inhibits the reuptake of serotonin into presynaptic neurons by blocking the serotonin transporter (SERT) with high potency and selectivity, increasing serotonin availability in the synaptic cleft. It has minimal affinity for dopamine, norepinephrine, muscarinic, histaminergic, or adrenergic receptors, resulting in a cleaner side-effect profile compared to older antidepressants. Therapeutic effects emerge after 2 to 4 weeks as serotonergic neurotransmission normalizes and downstream neuroplastic changes occur in mood-regulating brain circuits.

DOSAGE AND ADMINISTRATION

•      Major depression and anxiety disorders: 50mg once daily in the morning or evening

•      OCD and PTSD: may require 100 to 200mg per day under specialist guidance

•      Panic disorder: start at 25mg once daily for 1 week, then increase to 50mg

•      Always follow the dosage and titration schedule prescribed by your psychiatrist; may take 4 to 6 weeks for full effect

SIDE EFFECTS

Common:

•      Nausea and diarrhoea (most common, usually transient)

•      Headache and dizziness

•      Insomnia or somnolence

•      Dry mouth and sweating

Less common:

•      Sexual dysfunction: reduced libido, delayed orgasm, erectile dysfunction

•      Weight changes with prolonged use

•      Hyponatremia, particularly in elderly patients

•      Serotonin syndrome when combined with other serotonergic drugs (rare)

NOT RECOMMENDED IN

•      Concurrent use with MAO inhibitors (14-day washout required before and after sertraline)

•      Concurrent use with pimozide (serious QT prolongation risk)

•      Known hypersensitivity to sertraline

•      Children under 6 years of age

DRUG INTERACTIONS

MAO inhibitors are contraindicated; 14-day washout in both directions. Pimozide is absolutely contraindicated due to QT prolongation risk. Sertraline is a moderate CYP2D6 inhibitor and increases levels of tricyclics, some antipsychotics, and tamoxifen (reducing tamoxifen efficacy in breast cancer). NSAIDs and aspirin increase bleeding risk through platelet serotonin depletion. Warfarin anticoagulant effect may be enhanced; monitor INR.
