INDICATION

Actemra 162mg Inj is a humanized anti-interleukin-6 receptor monoclonal antibody indicated for the treatment of moderate to severe active rheumatoid arthritis in adults who have had an inadequate response to disease-modifying antirheumatic drugs (DMARDs). It is also indicated for giant cell arteritis (GCA) in adults, active systemic juvenile idiopathic arthritis (sJIA) and polyarticular juvenile idiopathic arthritis (pJIA) in patients 2 years and above, and severe or life-threatening cytokine release syndrome (CRS) induced by CAR-T cell therapy.

MECHANISM OF ACTION

Tocilizumab is a recombinant humanized IgG1 kappa monoclonal antibody that specifically binds to both soluble and membrane-bound interleukin-6 receptors (IL-6R), blocking IL-6 signaling. IL-6 is a pleiotropic pro-inflammatory cytokine produced by immune and non-immune cells that plays a central role in the pathogenesis of rheumatoid arthritis, GCA, and cytokine release syndrome by driving inflammation, autoantibody production, synovial hyperplasia, and joint destruction. By blocking the IL-6 receptor, tocilizumab prevents IL-6-mediated downstream signaling through JAK-STAT pathways, reducing the production of acute phase reactants, suppressing inflammatory cell recruitment, and halting progressive joint damage.

DOSAGE AND ADMINISTRATION

      Rheumatoid arthritis: 162mg subcutaneously every week, or every two weeks if used with DMARDs

      Giant cell arteritis: 162mg subcutaneously every week in combination with a tapering glucocorticoid course

      Administer by subcutaneous injection in the thigh, abdomen, or upper arm, alternating sites

      All dosing is under specialist supervision; screen for tuberculosis and hepatitis before initiating therapy

SIDE EFFECTS

Common:

      Nasopharyngitis and upper respiratory tract infections

      Hypertension

      Elevated total cholesterol, LDL, and triglycerides (monitor lipid profile)

      Injection site reactions: erythema, pruritus, and pain

Less common:

      Serious infections including bacterial, viral, fungal, and opportunistic infections (interrupt treatment if serious infection develops)

      Gastrointestinal perforation in patients with pre-existing diverticulitis (rare but serious)

      Elevated liver enzymes and hepatotoxicity

      Neutropenia and thrombocytopenia (monitor CBC regularly)

NOT RECOMMENDED IN

      Active, serious infections including tuberculosis (screen all patients before initiation)

      Known hypersensitivity to tocilizumab

      Neutrophil count below 2000/mm3, platelet count below 100,000/mm3, or ALT/AST above 1.5 times upper limit of normal before initiation

      Live vaccines should not be given during tocilizumab therapy; update vaccinations before starting

DRUG INTERACTIONS

Tocilizumab suppresses IL-6 signaling, which can normalize CYP450 enzyme activity suppressed during inflammation. This increases metabolism of drugs that are CYP450 substrates, potentially reducing their plasma levels; drugs particularly affected include statins, cyclosporine, theophylline, warfarin, and some oral contraceptives. Monitor drug levels and adjust doses as needed when starting or stopping tocilizumab. Avoid combination with other biological DMARDs such as TNF antagonists due to increased immunosuppression and infection risk.

STORAGE INSTRUCTIONS

Store between 2°C and 8°C (refrigerate). Protect from light. Do not freeze or shake. Allow to reach room temperature before injection. Keep out of reach of children.
