INDICATION

Aclovir 500mg Inj is an antiviral agent indicated for the treatment of severe herpes simplex virus (HSV) infections including herpes simplex encephalitis, neonatal herpes, and immunocompromised patients with disseminated or severe mucocutaneous HSV infections. It is also indicated for the treatment of varicella-zoster (chickenpox and shingles) in immunocompromised patients, herpes zoster encephalitis, and in organ transplant recipients requiring antiviral prophylaxis against herpesvirus infections. Intravenous administration is required when oral therapy is not feasible or the infection is life-threatening.

MECHANISM OF ACTION

Acyclovir is a synthetic purine nucleoside analogue that is selectively activated in virus-infected cells by viral thymidine kinase, which phosphorylates it to acyclovir monophosphate. Cellular enzymes then convert it to acyclovir triphosphate, which competitively inhibits viral DNA polymerase and is incorporated into the growing viral DNA chain, causing chain termination. Because viral thymidine kinase has much greater affinity for acyclovir than the host cell enzyme, the drug is selectively toxic to herpes-infected cells with minimal toxicity to normal host cells.

DOSAGE AND ADMINISTRATION

•      Herpes simplex encephalitis: 10mg/kg every 8 hours by slow intravenous infusion over 1 hour, for 14 to 21 days

•      Severe HSV in immunocompromised: 5mg/kg every 8 hours for 7 to 14 days

•      Varicella-zoster in immunocompromised: 10mg/kg every 8 hours for 7 to 10 days

•      Administer by trained healthcare professionals only; dose reduction required in renal impairment

SIDE EFFECTS

Common:

•      Nausea and vomiting

•      Headache

•      Elevation of creatinine and urea (renal function monitoring required)

•      Thrombophlebitis and inflammation at the injection site

Less common:

•      Encephalopathy (confusion, hallucinations, seizures) with high doses or in renal impairment

•      Acute renal failure from crystalline nephropathy (maintain adequate hydration)

•      Elevated liver enzymes

•      Anemia, neutropenia, and thrombocytopenia with prolonged use

NOT RECOMMENDED IN

•      Known hypersensitivity to acyclovir or valacyclovir

•      Administer slowly (over at least 1 hour) to reduce crystalluria and nephrotoxicity

•      Ensure adequate hydration before and during infusion

•      Dose adjustment mandatory in renal impairment

DRUG INTERACTIONS

Probenecid reduces renal tubular secretion of acyclovir, increasing plasma levels; monitor for toxicity. Cimetidine also reduces acyclovir renal clearance. Concurrent nephrotoxic agents such as aminoglycosides, amphotericin B, and ciclosporin increase nephrotoxicity risk; monitor renal function closely. Acyclovir may increase plasma levels of mycophenolate in transplant recipients; monitor accordingly. Zidovudine combined with acyclovir may cause profound drowsiness and lethargy.

STORAGE INSTRUCTIONS

Store unconstituted vials below 25°C away from direct light. Use reconstituted solution within 12 hours. Do not refrigerate reconstituted solution (crystals may form). Keep out of reach of children.
